Résumé
Cet article décrit les caractéristiques endoscopiques, cliniques et histo-pathologiques de 33 papillomes œsophagiens diagnostiqués à Lausanne entre 1973 et 1988, en les comparant aux données de la littérature (103 papillomes). Le papillome de l’cesophage est une lésion rare, de faible dimension, le plus souvent unique et asymptomatique, sans rapport avec le reflux gastro-œsophagien. Les potentialités malignes de la lésion sont apparemment nulles, l’évolutivité faible et la récidive inexistante après exérèse complète. Les signes histologiques d’une lésion virale, recherchés par la technique des sondes biotinylées, sont peu fiables et la quantité de virus 31-33-35 décelée dans notre série est faible. En présence d’une lésion de ce type, le rôle essentiel de l’endoscopiste consiste à exclure la forme rare d’un carcinome « précoce » de type papillaire grâce à une coloration vitale et une biopsie-exérèse sélective. L’impact clinique du papillome œsophagien sur la carcinogenèse épidermoïde est nulle en Occident, étant donné la rareté de la lésion.
Summary
This article describes the endoscopic, clinical and histopathological characteristics of 33 esophageal papillomas diagnosed in Lausanne between 1973 and 1988, and compares them to literature data (103 papillomas). Papilloma of the esophagus is a small-sized, rare lesion, most often isolated and asymptomatic, and non-related to gastroesophageal reflux. The lesion’s potential for malignancy is apparently nil. The evolution is low-grade and recurrences never occur following complete excision. Histological signs of viral infection sought by biotin probes are not reliable, and the quantity of virus 31-33-35 detected in our series is small. In presence of lesions of this type, the chief role of the endoscopist consists in excluding the rare form of papillary-type early carcinoma with the use of vital staining and selective bioptic excision. The clinical impact of esophageal papilloma on epidermoid carcinogenesis is nil in the West, given the rarity of the lesion.

Este artéculo describe las caracterésticas endoscópicas, clénicas e histopatológicas de 3 papilomas esofagianos diagnosticados en Lausana entre 1973 y 1988, comparándolos con los datos de la literatura (103 papilomas). El papiloma del esófago es una lesión rara, de escasas dimensiones, en la mayor parte de veces es único y asintomático, sin relación con el reflujo gastrœsofagiano. Las potencialidades malignas de la lesión son aparentemente nulas, la evolución limitada y la recidiva inexistente tras exéresis completa. Los signos histológicas de una lesión viral, investigados mediante la técnica de sondas botiniladas, son poco fiables y la cantidad de virus 31-33-35 descubierta en nuestra serie es pequeña. En presencia de una lesión de este tipo, la función esencial del endoscopista consiste en excluir la forma rara de un carcinoma « precoz » de tipo papilar, gracias a una coloración vital y una biopsia-exéresis selectiva. El impacto clénico del papiloma esofagiano sobre la carcinogénesis epidermica es nulo en Occidente, dada la rareza de la lesión.
