41 patients (19 femmes, 22 hommes, âge moyen 74 ans (41–93) présentant un ictère obstructif d’origine extra-hépatique ont été investigués par ultrasonographie transcutanée (US), ultrasonographie endoscopique (USE), cholangiowirsungographie rétrograde endoscopique (CPRE) au cours des 24 h. qui ont suivi l’admission à l’hôpital. L’USE fut toujours réalisée après un examen US et immédiatement avant la CPRE, examen considéré dans cette étude comme le «gold standard». Les données de l’USE ont été documentées selon le protocole de données préétablies, en double insu, par rapport aux résultats de la CPRE. La dilatation de la voie biliaire principale (VBP) a été mise en évidence par les trois méthodes. Les calculs de la VBP, responsables de l’obstruction biliaire ont été démontrés par USE dans 15/16 cas et seulement chez 7 patients par examen US. Chez 25 malades une pathologie maligne a été identifiée tant par USE que par CPRE et le niveau de l’obstruction biliaire a été déterminé correctement, alors qu’à l’examen US le diagnostic n’a été porté respectivement que dans 17 cas (68%) et 20 cas (80%). En comparaison avec les résultats de la CPRE, l’USE a procuré une image correcte de la tumeur et a permis sa stadification locale chez tous les malades. Chez 8 malades sur 10 patients opérés, les dimensions de la tumeur étaient correctement évaluées par l’USE pré-opératoire, tandis que l’US avait tendance à sous-estimer l’extension tumorale. Dans les cas de patients porteurs d’une obstruction tumorale de la VBP, l’USE a contribué de façon utile à la prise de décision d’un traitement chirurgical ou palliatif endoscopique. L’USE a par conséquent un impact sur la stratégie thérapeutique des patients présentant une pathologie obstructive extra-hépatique.
Summary
41 patients (19 females, 22 males, mean age 74 (41–93) years) with extrahepatic obstructive jaundice were evaluated by transcutaneous ultrasonography (US), endoscopic ultrasonography (EUS) and endoscopic retrograde cholangiopancreatography (ERCP) within 24 hours of hospital admission. EUS was always performed after US and immediately prior to ERCP, the latter considered the gold standard for this study. Findings of EUS were documented according to a preset data protocol in a blind fashion with respect to ERCP results. Common bile duct (CBD) dilation was demonstrated by all three methods. Stones in the CBD causing obstruction were demonstrated by EUS in 15 out of 16 patients but in only seven by US. In 25 patients underlying malignant disease was identified by EUS as well as ERCP and the level of biliary obstruction was determined correctly, which was possible with US in only 17 (68%) and 20 (80%) cases respectively. In comparison to ERCP, EUS provided a direct image of the tumor and allowed for local staging in all patients. In 8 out of 10 operated patients tumor size was correctly determined by preoperative EUS, while US tended to underestimate tumor extension. In patients with CBD obstruction caused by tumors EUS was helpful in deciding whether operative resection or palliative endoscopic management should be performed. EUS thus may have an impact on therapeutic strategy in patients with extrahepatic obstructive jaundice.
