Résumé

Devant l’essor des techniques de dilatation œsophagiennes afin de guider le choix des praticiens, le matériel actuellement existant (olives, bougies, cathéter-ballonnet, dilatateur pneumatique, fil guide) est décrit. Les études comparatives effectuées et la revue de la littérature font apparaître une nette supériorité des bougies (Savary-Gilliard, Biomed, Célestin) sur l’appareil d’Eder-Puestow à olives. Les bougies de Savary-Gilliard et Biomed semblent d’efficacité comparable. La preuve de 1’intérêt des cathéterballonnet par rapport aux bougies n’apparaît pas clairement dans les études actuellement publiées.

Les dilateurs pneumatiques sont efficaces dans le traitement de l’achialasie (Rider-Moeller, Witzel). Les fils guides doivent être à flexibilité progressive.

Summary

In front of the rise of the esophageal dilatation technics, in order to guide the choice of the practitioners, the material existing now (olives, bougies, catheter balloon, pneumatic dilators, guide wires) is described.

The comparative studies carried out, and the review of the literature make clearly appear the superiority of the bougies (Savary-Guilliard, Biomed, Celestin) to the EP apparatus.

The Savary-Guilliard and Biomed bougies seem to leave quite an equal efficiency. The proof of the interest of the catheter-balloon does not appear clearly with regard to the bougies, in the studies published up to now.

The pneumatic dilators are efficient in the Achalasia treatment. (Rider-Moeller, Witzel).

The guide wires must have a progressive flexibility.
