A policy for conservative management of pedunculated malignant polyps in the colon

Résumé

L’endoscopiste peut suspecter la malignité d’un polype colique d’après son aspect irrégulier/ulcéré, ou du fait de l’induration de la tête du polype ou de son pédicule.

En cas de suspicion de malignité, le pédicule doit être correctement réséqué afin de permettre des sections histologiques multiples à ce niveau. Le pathologiste apprécie à la fois le caractère complet de la résection et le degré de différenciation d’un adénocarcinome invasif. Le traitement chirurgical est normalement réservé aux cancers peu différentiés ou aux envahissements néoplasiques étendus jusqu’à quelques milimètres de la tranche de section électro-chirurgicale. La même ligne de conduite est applicable aux carcinomes polypoïdes. Au moment de la polypectomie ou au cours des deux semaines qui suivent, un marquage à l’encre de Chine est effectué afin d’identifier le siège de la polypectomie et sa reconnaissance lors d’examens ultérieurs. La base de section est contrôlée après 2 mois et ensuite après un an, avant que le patient n’entre dans le cadre de la surveillance classique postpolypectomie.

Summary

The endoscopist may suspect malignancy from irregular/ulcerated configuration of a polyp or induration in its head or stalk.

When malignancy is suspected an adequate excision of stalk must be achieved to allow multiple histological sections through this region. The histologist records both completeness of excision and degree of differenciation of the invasive adeno-carcinoma. Surgery is normally reserved for poorly differentiated cancers or where invasion reaches within a few millimetres of the electrosurgical transection. The same policy is applied to polypoid carcinomas. At the time of polypectomy or within 2 weeks afterwards an Indian ink tattoo is made to identify the polypectomy site for follow-up purposes. The base is reinspected at 2 months and then at 1 year before assuming normal post-polypectomy surveillance.
