Résumé
L’expérience de prélèvements cytologiques du tractus digestif supérieur par voie endoscopique, depuis son début, et portant sur une période de six ans est revue. Quatre types de pathologies ont été revues: la mycose œsophagienne (n=23), le cancer œsophagien (n=23), le cancer gastrique (n=96), la giardiase duodénale (n=10). L’examen cytologique donne des résultats rapides et spécifiques dans les différents groupes. La comparaison des cancers gastriques et d’un groupe ďulcères gastriques révèle l’absence de faux positifs.
La sensibilité de l’examen cytologique des lésions néoplasiques est proche de celle des prélèvements biopsiques, améliorant le score diagnostique global (+24% pour l’œsophage — + 9% pour l’estomac). La comparaison de deux types de prélèvements cytologiques révèle une plus grande richesse cellulaire et une positivité plus élevée des empreintes par rapport aux brossages. Enfin, la comparaison de deux périodes successives montre une amélioration des résultats après une période d’apprentissage.
Summary
The experience of cytological samples of the upper digestive tract’ by endoscopy since its beginning and over a period of 6 years has been reviewed. 4 types of pathologies were examined: oesophageal mycosis (n=23), oesophageal cancer (n=23), gastric cancer (n=96), duodenal giardiasis (n=10). Cytological examination gives rapid and specific results in the different groups. The comparison of gastric cancers and a group of gastric ulcers reveals the absence of false positives.
The sensitivity of the cytological examination of neoplastic lesions is about the same as for biopsic samples increasing the overall diagnostic score (+24% for the oesophagus -+9% for the stomach). The comparison of 2 types of cytological samples reveals a greater cell richness and a higher level of positive results by imprints compared to brushing. Finally the comparison of 2 successive periods shows that results improve after a training period.
