Ulcères de l’œsophage secondaires à la prise d’amoxycilline (1 cas) et de méquitazine (1 cas)

RéSUMé

Deux cas d’ulcères œsophagiens iatrogènes sont rapportés, secondaires à la prise d’amoxycilline d’une part, et de méquitazine d’autre part. Dans les deux cas la prise du médicament est stéréotypée (à sec, à jeun, patient alité) et la clinique bruyante. L’endoscopie confirme le diagnostic et montre une fois des ulcérations du tiers inférieur, et une fois un ulcère rond du tiers moyen sus-aortique. La toxicité œsophagienne de ces médicaments n’a jamais été rapportée. Son mécanisme est différent de celui concernant la doxycycline, puisque nos deux médicaments sont caractérisés par le caractère alcalin de leur molécule. La gastro-toxicité de l’amoxycilline est également possible et discutée dans notre première observation.

Summary

We report two cases of oesophageal ulcers induced by amoxycilline and méquitazine therapy. In both cases the drugs were taken without water, without eating and whilst lying down. The clinical symptoms were typical. Endoscopy confirms diagnosis and reveals in one case ulcers of the lower oesophagus and in the other case one small round ulcer of the mid oesophagus. Oesophageal toxicity due to these drugs has so far never been reported. This mecanism of toxicity is different from the one characterizing doxyciclin as both amoxycillin and méquitazine have an alcaline pH. Gaslro-toxicity of amoxycillin is also possible and is discussed in our first case.
