Une zone pâle et veloutée de 0,7 cm de diamètre fut observée dans le sigmoïde en cours de coloscopie chez un patient exempt de maladie inflammatoire du côlon ou d’autres lésions macroscopiques.
La zone tissulaire a été réséquée ð l’anse diathermique.
L’examen histologique a montré une dysplasie cellulaire de la portion la plus superficielle de la muqueuse, entourant un petit foyer de cancérisation limité ð la muqueuse.
Ce cas permet d’avancer quelques hypothèses sur la cancérogenèse du côlon et montre la nécessité d’exécuter des biopsies sur n’importe quelle anomalie, même si minime, de la muqueuse colo-rectale.
Summary
A pale velvety area (0.7 cm in diameter) was observed by colonoscopy in the sigmoid colon of a patient without inflammatory bowel disease nor other macroscopic lesions in the remaining large bowel.
The area was excised by dyathermic snare and, at histology, it showed dysplasia of the upper portion of the mucosa with a small focus of intramucosal carcinoma.
This report leads to some speculations on the colon carcinogenesis and indicates the need to obtain biopsies from any macroscopical abnormality, even if mild, of the recto- colonic mucosa.
