Le rôle de l’endoscopie dans la maladie cœliaque de l’enfant

Role of endoscopy in childhood coeliac disease

RÉSUMÉ

Les auteurs ont effectué une endoscopic chez 47 enfants (limites d’âge : 2 et 15 ans) atteints de maladie cœliaque précédemment diagnostiquée par la méthode de la biopsie ð la sonde (aspiration-section) ð un âge moyen ne dépassant pas 2 ans.

Les résultats endoscopiques et histologiques étaient normaux chez 9 enfants soumis ð l’endoscopie immédiatement après arrêt du régime sans gluten.

Chez les 38 autres enfants, étudiés après réintroduction du gluten en fin de période de régime, les données endoscopiques étaient pathologiques chez 37 % d’entre eux et l’histologie révéla des anomalies muqueuses dans 95 % des cas. Ces anomalies, évaluées tant en degré qu’en « score », se répartissent dans les cas de la façon suivante : atrophie villositaire partielle légère (22,2 %), atrophie villositaire partielle sévère (52,8 %) et subatrophie totale (25 %).

Les auteurs pensent que dans la maladie cœliaque de l’enfant, l’endoscopie peut chaque fois être utilisée après réintroduction du gluten afin de confirmer le diagnostic, et révèle le degré d’anomalie muqueuse en réponse ð la réintroduction du gluten dans le régime.

Summary

The AA.carried out endoscopy in 47 children (age range 2 to 15 years) with coeliac disease, which had been previously diagnosed by aspiration capsule biopsy at mean age of less than 2 years.

Endoscopic and histologic results were normal in 9 children who underwent endoscopy after gluten free diet.

In the other 38 children observed after challenge with gluten following G.F.D., pathologic endoscopic findings were seen in 37 % and histologie damage in 95 % of the cases. This damage, evaluated both in grades and in score, consisted of slight partial villous atrophy in 22.2 % severe partial villous atrophy in 52.8 % and subtotal villous atrophy in 25 % of the cases.

The AA. believe that in childhood coeliac disease endoscopy should always be used after challenge to confirm the diagnosis and reveal the degree of mucosal damage in response to the addition of gluten to the diet.
