Endoscopic methodology and the results of drug trials on the oeso-gastro-duodenal pathology

Résumé

La nécessité de recourir ð des essais thérapeutiques contrôlés en double insu pour apprécier l’efficacité des traitements portant sur les affections du tractus digestif supérieur est actuellement unanimement admise. Lors de ces essais, l’endoscopie occupe une place importante, d’une part pour définir les critères d’inclusion, d’autre part pour évaluer l’efficacité du traitement, par exemple la cicatrisation d’un ulcère. Cependant en dépit de progrès méthodologiques importants dans la conduite de ces essais, l’analyse de la littérature révèle de nombreuses divergences dans les résultats de telles études. Le but de cet Editorial est de montrer ð l’aide de quelques exemples dans quelle mesure les paramètres endoscopiques retenus dans ces essais sont susceptibles d’influencer les résultats et d’expliquer les discordances.

Abstract

The necessity of using controlled double blind therapeutic tests to appreciate the efficiency of treatments on illnesses of the upper digestive tract has now been unanimously agreed. During these trials, endoscopy occupies an important place, on the one hand to define the criteria of inclusion, on the other hand to evaluate the efficiency of the treatment, for example the healing of an ulcer. However inspite of the great methodological progress in carrying out these trials, the analysis of the literature reveals many divergences in the results of such studies. The aim of this editorial is to show with few examples to what extent the endoscopic parameters used in these trials might influence the results and explain the discrepancies.
