Résumé
L’essor de la micro-chirurgie endoscopique du pharyngo-larynx au cours de ces vingt dernières années est dû essentiellement à l’apport du laser CO2 dans le traitement des pathologies tumorales et au développement de la phonochirurgie dans les atteintes fonctionnelles de la voix.
En pathologie tumorale, le laser a offert la possibilité de procéder à l’exérèse endoscopique curative de certains carcinomes ou de certaines tumeurs bénignes du larynx ou du pharynx. Dans ce type de chirurgie, le respect rigoureux des indications ainsi qu’une connaissance approfondie de la morphologie lésionnelle des cancers précoces sont essentiels.
De 1976 à 1990, 1 251 micro-pharyngo-laryngoscopies en suspension ont été effectuées dans le service ORL du CHUV. La technique, les indications ainsi que le taux de complications liées à cet examen sont passés en revue.
Summary
The rapid development of endoscopic microsurgery of the pharyngo-larynx in these past twenty years is due essentially to the contribution of CO2 laser in the treatment of tumoral pathology and the development of phonosurgery in. the functional disorders of the voice.
In tumoral pathology, the curative endoscopic exeresis of certain carcinomas or of some benign tumours of the larynx and the pharynx, has been made possible with laser. In this type of surgery, a rigourous regard on the indications as well as a detailed knowledge of the morphology of cancer lesions is mandatory.
From 1976 to 1990, some 1 251 suspension micropharyngolaryngoscopies were performed in the Ear, Nose, Throat clinic of the CHUV. The technique, the indications as well as the complications arising from this examination are reviewed.

El progreso de la micro-cirugía endoscópica de la faringo-laringe en el curso de estos veinte últimos años es debido esencialmente al aporte del laser CO2 en el tratamiento de las patologías tumorales y al desarrollo de la fonocirugía en las lesionales funcionales de la voz. En patología tumoral, el laser ha ofrecido la posibilidad de proceder a la exéresis endoscópica curativa de ciertos carcinomas o de ciertos tumores benignos de la laringe o de la faringe. En este tipo de cirugía, el respeto riguroso de las indicaciones así como un conocimiento profundo de la morfología lesional de los cánceres precoces, son esenciales.
Zusammenfassung
Der Aufschwung der endoskopischen Mikrochirurgie der Pharynx-Larynx-Region im Verlauf der letzten 20 Jahre ist vor allem im Zusammenhang mit der Anwendung des CO2-Lasers in der Behandlung von Tumoren sowie in der Entwicklung der Phonochirurgie bei funktionellen Krankheiten bei Stimmbildung zu sehen. Bei bestimmten Karzizomen oder benignen Tumoren in Larynx oder Pharynx bietet der Laser die Möglichkeit einer kurativen endoskopischen bietet der Laser die Möglichkeit einer kurativen endoskopischen Entfernung. Für diese Art der Behandlung sind die strikte Beachtung von Indikationen sowie die profunde Kenntnis der Morphologie von Frühkarzinomen unerläßlich.
Von 1976 bis 1990 wurden 1251 Micro-Pharyngo-Laryngoskopien in der HNO-Klinik. der Universitätsklinik Lausanne vorgenommen. Technik, Indikationen sowie die Komplikationen bei dieser Methode werden dargestellt.

Il progresso della microchirurgia endoscopica applicata alla patologia della faringe e della laringe avvenuto negli ultimi anni è specialmente legato all’apporto dato dalla utilizzazione del laser a CO2 nel trattamento dei tumori e allo sviluppo della fonochirurgia nelle affezioni funzionali della voce. In patologia oncologica, il laser ha permesso di eseguire exeresi endoscopiche curative di carcinomi o tumori benigni localizzati a livello faringeo-laringeo. Per poter effettuare questo tipo di terapia è essenziale un rispetto rigoroso delle indicazioni ed una conoscenza approfondita dell’aspetto morfologico delle lesioni neoplastiche nella loro forma iniziale.
Dal 1976 al 1990 sono state eseguite 1 251 micro-faringo-laringoscopie presso il servizio ORL del CHUV. Nell’articolo sono riportate la tecnica, le indicazioni ed il tasso di complicanze legate a questa metodica.
