Proper endoscopic surveillance of Barrett’s esophagus

RéSUMé

Bien que l’œsophage de Barrett (OE) porte le risque reconnu d’une évolution adénocarcinomateuse, la valeur du dépistage endoscopique, la fréquence et les modalités des biopsies endoscopiques de même que l’interprétation des précurseurs du cancer sont encore discutés. En effet, comme la surveillance de tous les patients porteurs d’œsophage de Barrett est onéreuse et demande du temps, les programmes de surveillance devraient être orientés sur des sous-groupes de patients davantage susceptibles de développer un cancer, à l’exclusion des patients ne pouvant être soumis à la chirurgie.

Par conséquent, il semblerait raisonnable d’établir une politique de surveillance endoscopique intensive chaque 3 à 6 mois pour les patients porteurs d’un long segment de muqueuse métaplasique ou de modifications dysplasiques. Les biopsies sous vision directe sont préférables aux biopsies à l’aveugle afin d’obtenir des échantillons multiples en quadrant, à 2 cm d’intervalle, tout le long du segment d’œsophagc de Barrett.

Summary

Although Barrett’s esophagus (BE) carries a defined risk of adenocarcinoma, the value of endoscopie screening, timing and modality of endoscopically obtained biopsy specimens and interpretation of cancer precurscor are still under discussion. As endoscopie surveillance in all patients with BE is expensive and time consuming, surveillance programme shoud be focussed on sub- groups of subjects who are more likely to develop carcinoma excluding patients with obvious unfitness for surgery.

Therefore it seems a reasonable policy to submit to intensive endoscopie control every 3- 6 months subjects with long segment of metaplastic mucosa and/or dysplastic changes. Pinch biopsies under direct vision must be preferred to blind biopsies obtaining multiple quadrantic specimens at 2 cm intervals along BE segment.
