Experimental findings in Barrett’s esophagus

RéSUMé

Les études expérimentales de Bremner, pionnier en la matière et le protocole mis au point dans la Division de Chirurgie Digestive de la Faculté de Médecine de l’Université de Sâo Paulo, ont permis de reproduire chez le chien, un épithélium en métaplasie cylindrique sur une surface œsophagienne dont la muqueuse a été expérimentalement réséquée et mise en présence d’un reflux gastro-œsophagien.

Sur une série analysée de 50 cas, 44 furent soumis à un traitement chirurgical, avec surveillance endoscopique semestrielle, qui démontra l’arrêt de l’expansion de l’épithélium cylindrique sur la muqueuse œsophagienne grâce au contrôle chirurgical du reflux.

Ces observations soutiennent l’hypothèse de l’origine acquise de l’endobrachyœsophage et montrent une corrélation clinique significative avec les données de l’expérimentation.

Summary

The pioneering Bremner’s experimental studies and the protocol developped on the Division of Surgery of the Digestive System of the University of Sao Paulo Medical School reproduced the Barrett’s epithelium, in the presence of gastroesophageal reflux, in the area of esophageal mucosa resection, in the dog.

Analysing 50 cases, 44 of them managed surgically, with semiannual endoscopie followup, it was observed the blocking of spread of columnar epithelium on the esophageal mucosa, by surgical control of the reflux.

These data provide substantial support to the acquired origin of Barretts epithelium, significant clinical correlation with the experimental findings.
