Résumé
Bien qu’il puisse théoriquement se rencontrer dans des pathologies très variées, le granulome epithélioïde constitue dans nos contrées, une lésion histologique pratiquement pathognomonique permettant de différencier la maladie de Crohn des autres affections inflammatoires du tractus digestif.
De nombreuses études ont été consacrees à son identification sur des biopsies pratiquées en endoscopie et à la recherche des conditions optimales de sa découverte.
Des travaux récents, ressort que la probabilité de le détecter augmente nettement si les biopsies sont profondes, multiples, prélevées en site pathologique — ulcérations aphtoïdes surtout — et examinées par des pathologistes expérimentés pratiquant des coupes semi-sériées.
Par ailleurs, il existe des variations régionales dans la distribution des granulomes ; c’est ainsi que les biopsies rectales réalisées en muqueuse saine peuvent offrir un certain intérêt diagnostique. Le microgranulome par contre, entité décrit plus tardivement, offre moins ďintérêt car il peut se voir dans de nombreuses autres affections inflammatoires du tube digestif.
Bien qu’il soit plus fréquent dans la maladie de Crohn, son apport diagnostique n’est pas prouvé.
Summary
Although epithelioid granuloma theoretically may be present in quite varied pathologies, in our regions this histological lesion is virtually pathognomonic, allowing differentiation between Crohn’s disease and other inflammatory disorders of the digestive tract.
Many studies have been devoted to granuloma identification in endoscopic biopsies, and determining optimal detection conditions.
Recent studies have shown that detection probability increases sharply if biopsies are deep, multiple, taken from pathological sites — especially aphthoid ulcerations — and examined by experienced pathologists using semi-serial section techniques.
Regional variations in granuloma distribution exist: this is why rectal biopsies on healthy mucosa can be of diagnostic interest. In contrast, a more recently described entity, microgranuloma, is much less useful as a diagnostic tool due to its presence in numerous other inflammatory disorders of the digestive tract.
Although it is more frequent in Crohn’s disease, its diagnostic relevance has yet to be established.

Aunque es posible encontrarlo, en teoría, en patologías de muy diversa índole, el granuloma epitelioide constituye, en nuestro medio, una lesion histológica practicamente patognomonica que permite diferenciar la enfermedad de Crohn de otras afecciones inflamatorias del tracto digestivo.
Son numerosos los trabajos dedicados a la identificacidn de dichos granulomas en las biopsias endoscópicas asi como a la investigaciónde las condiciones optimas de la manera de hallarlos.
En los estudios recientes se destaca que la posibilidad de detección aumenta netamente si las biopsias son profundas, multiples, obtenidas en areas patologicas (ulceraciones aftoides muy particularmente) y si son examinadas por patólogos experimentados que practican cortes semiseriados.
Además se sabre que existen variaciones regionales en la distributión de los granulomas ; es por ello que las biopsias rectales sobre mucosa sana pueden ofrecer un cierto interes diagnostico.
Por contra, el microgranuloma, entidad descrita posteriormente, ofrece menos interes ya que puede verse en numerosas enfermedades inflamatorias intestinales otras que la de Crohn.
A pesar de su mayor frecuencia en la enfermedad de Crohn no se ha podido probar su validez en el diagnóstico.
