Résumé
Nous rapportons le cas d’une femme âgée de 95 ans ayant eu une extraction endoscopique d’un volumineux calcul vésiculaire enclavé dans le bulbe, provoquant une occlusion. Un mois plus tard, le calcul avait disparu, ayant peut-être été dissous par les sucs gastriques.
Nous rappelons le rôle primordial de l’endoscopie dans le syndrome de Bouveret tant à visée diagnostique que thérapeutique, l’intérêt de la mobilisation du calcul vers l’antre même si son extraction première paraît parfois impossible.
La mortalité des obstructions duodénales par lithiase est de l’ordre de 15 à 30 % [1]. L’extraction endoscopique peut en améliorer le pronostic.
Summary
We relate the case of a woman, aged 95, who had an endoscopic extraction of gallblader calculus, which moved into the bulb, causing a stoppage.
The calculus left in the stomach because of its size, was not found one month later because it had spontaneously dissolved. We must bear in mind the interest of endoscopy in Bouveret’s syndrome for diagnosis and treatment. It is interesting to move the calcul if its complete extraction is impossible.
Death rate is about 15 to 30 % for duodenum stoppage by cholelithiasis.
The endoscopic extraction can improve prognosis.
