Conclusion
L’entéroscopie poussée peut être utile comme étape diagnostique ultérieure dans les cas de diarrhée ou de malabsorption, car elle permet d’étudier des images indicatives d’une malabsorption. En présence d’anomalies muqueuses même faibles, l’endoscopiste peut diriger la pince à biopsie vers ces régions qui pourraient être les plus intéressantes pour le diagnostic.
Nous avons démontré l’efficacité diagnostique de l’histologie jéjunale par rapport à l’histologie duodénale, et nous pensons que l’entéroscopie ne doit pas se substituer à l’endoscopie haute avec biopsie dans l’exploration de ces patients. L’entéroscopie devrait être réservée à des cas sélectionnés c’est-à-dire pour les patients chez lesquels les résultats de l’endoscopie et de l’histologie duodénale ne sont pas concluants et pourrait être proposée comme un examen de première intention quand on suspecte une maladie du grêle à distribution focale.
