Résumé
Depuis 5 ans, nous avons étudié en manométrie œsophagienne, 7 cas de mégaœsophage idiopathique infantile. Dans toutes les observations, même s’il n’existait pas de distension œsophagienne radiologique, nous avons noté des anomalies spécifiques: hypertonie et achalasie du sphincter inférieur de l’œsophage (SIO) et dyskinésie caractéristique du corps de l’œsophage. La manométrie nous a permis d’éliminer les autres causes de mégaœsophage infantile. Une intervention de Heller modifiée, suivie d’un procédé anti-reflux, a été pratiquée de premiére intention chez 3 patients avec de bons résultats dans 2 cas. La persistance des symptômes dans le troisiéme, a motivé une nouvelle cardiomyotomie. Les 4 derniers malades ont été traités médicalement par Nifédipine. Le premier a été perdu de vue. Les résultats ont été excellents dans 2 cas, mais de façon transitoire (5 mois) chez l’un d’eux âgé d’un an. Dans le troisiéme, les signes cliniques et manométriques se sont aggravés, malgré l’augmentation des doses de Nifédipine. Ces 3 patients ont été finalement opérés. Les études manométriques post-opératoires montraient un effrondrement du tonus du SIO, mais une persistance de l’achalasie et une régression modérée de la dyskinésie œsophagienne. Lorsqu’il est efficace, le traitement médical permet aux patients d’affronter l’acte chirurgical dans d’excellentes conditions nutritionnelles.
La manométrie représente un excellent test de dépistage et de surveillance du mégaœsophage idiopathique, chez l’enfant comme chez l’adulte.
Summary
Over the past 5 years, we performed oesophageal manometry in 7 children afflicted with oesophageal achalasia. In all cases, including the patients without radiological oesophageal distension, we found the caracteristic manométric aspect of achalasia: high pressure and achalasia of the lower oesophageal sphincter (LES), specific disorders of the oesophageal motility. Manometry allowed us to eliminate other causes of megaoesophagus in childhood. A transabdominal modified Heller’s operation followed by an additional anti-reflux procedure was performed directly in 3 children with good results in 2; the persisting symptoms in the third required a second oesophagomyotomy. The 4 last patients received a Nifédipine therapy. I child was lost to follow-up. We obtained good results in 2, but the manométric improvement was transient (5 mths) in a 1 year old boy. In the 3rd case, symptoms and manométrie data did not improve, despite Nifédipine high dose. Heller’s procedure was performed at last in all. In the post-operative manométrie studies, LES pressure decreased, but achalasia and lessening motor disorders persisted. When it was efficient, the medical treatment permitted the patients to recover better nutritional conditions before surgery.
Manometry represents a good laboratory test for diagnosis and follow up of achalasia in childhood.
