Résumé
Nous avons étudié les renseignements fournis par la pH métrie de longue durée (18–24 h) chez 43 nourrissons et enfants (9 témoins, 16 malades à Symptomatologie digestive de reflux et 18 enfants présentant des problèmes respiratoires chroniques et un reflux gastro-œsophagien authentifié par au moins un test diagnostique positif: TOGD, scintigraphie et/ou endoscopie). La pH métrie est analysée sous l’angle de 10 paramètres: pourcentage de temps passé à pH < 4, nombre de reflux/heure, nombre de reflux de plus de 5 minutes par heure, durée de reflux le plus long: sur 24 h et sur la nuit, la durée moyenne de reflux la nuit et un systéme de score post-prandial.

Les résultats des pH métries des malades digestifs diffèrent des témoins de façon significative pour tous les paramétres sauf la durée moyenne de reflux nocturne.

Les résultats des pH métries des « respiratoires » diffèrent des témoins de façon significative pour tous les paramètres sauf le systéme de score post-prandial.

Les résultats moyens des paramétres étudiés dans les différents groupes d’enfants ont des valeurs tout à fait comparables à celles obtenues par les auteurs utilisant le même matériel.

La fixation de limite de normalité permet de calculer les spécificités et sensibilités des différents paramétres chez les témoins, les digestifs, et de montrer que les paramétres analysant l’ensemble du tracé sont les plus sensibles chez les « digestifs » alors que chez les « respiratoires » les paramétres nocturnes sont les plus discriminants.

Le systéme de score post-prandial ne s’adapte, dans notre étude, qu’au seul groupe de malades digestifs (où une corrélation entre score et pourcentage de temps passé à pH 4 existe tant la nuit que le jour).

La durée de reflux nocturne ainsi que le nombre de reflux de plus de 5 minutes par heure la nuit semblent avoir un intérêt pronostique chez les malades « respiratoires ».
Summary
We have studied the data of continuous monitoring of distal oesophagus pH in 43 infants and children (9 controls, 16 patients) with digestive symptoms and 18 patients with chronic respiratory symptoms and a gastro-oesophageal reflux proved by at least one positive diagnostic test: baryum oesophagus, scintigraphy andlor endoscopy. 10 parameters of pH monitoring are analysed on the 24 hours and night recordings: percent of monitored time with pH < 4.0 number of episodes of reflux per monitored hour, number of reflux over 5 minutes duration, duration of the longest episode of reflux. A post prandial score and on night time the mean duration of reflux have also been established:

The results pH monitoring of the « digestive » patients are significantly different from controls for all parameters except for the mean duration of nocturnal reflux.

The results of pH monitoring of the « respiratory » patients are significantly different from controls for all the parameters except for the post prandial score.

The mean of the parameters studied in all the subgroups of patients are similar to the figures recorded in the literature with the same methods.

Specificity and sensitivity of each variable are calculated for controls and « digestive » patients. Long recording parameters are more sensitive among « digestive » patients while nocturnal parameters are more specifics in respiratory patients.

A positive correlation exists between post prandial score and percentage of time spent below pH 4 in digestive patients only.

In « respiratory » patients the mean duration of nocturnal reflux and the number of reflux exceeding 5 minutes per hour have a prognostic value.
