Résumé

La présente étude est consacrée à 1’évaluation du rôle actuel de la laparoscopie dans le diagnostic de carcinome hépatocellulaire, sur base d’une série de 157 patients consécutifs.

La laparoscopie a été utilisée dans le diagnostic initial, chez un petit nombre de patients, alors que la majorité des cas ont été explorés dans un premier temps par la biopsie echoguidée à l’aiguille fine. Les autres indications de la laparoscopie dans la présente série ont été: la correction des faux négatifs de la biopsie échoguidée; la stadification pré-chirurgicale (15/157 patients) et enfin, le contrôle de positivité de la biopsie échoguidée dans la phase initiale de son utilisation dans notre service.

En conclusion, la laparoscopie n’est plus la technique invasive de premier choix pour le diagnostic histo-pathologique de carcinome hépato-cellulaire; pourtant, elle joue encore un rôle tres important dans des cas sélectionnés. L’intégration entre résultats de la laparoscopie et de la biopsie échoguidée fournit les meilleurs résultats diagnostiques.

Summary

The paper evaluates the actual role of laparoscopy for diagnosis of HCC, on the basis of a series of 157 patients consecutively observed.

A small number of laparoscopics has been carried out for the initial diagnosis; in most cases on the contrary US guided fine needle aspiration biopsy has been used. Laparoscopy has been employed to correct false negatives of ultrasound guided biopsy; to presurgical staging (15/157 patients) and, finally, to check the true positivity of ultrasound guided biopsy in the first phase of its application.

In conclusion laparoscopy is no longer the first choice invasive technique for pathological diagnosis of HCC; however it maintains a fundamental role in selected cases. The integration of laparoscopy and ultrasound guided biopsy gives the best level of diagnostic accuracy.

Resumen

El presente estudio está consagrado a la evaluación del papel actual de la laparoscopia en el diagnóstico del carcinoma hepatocelular, y se basa en el análisis de una serie de 157 pacientes consecutivos.

La laparoscopia fué utilizada como diagnóstico inicial en un pequeño número de pacientes, habiendose explorado la mayoría en primera intención por biopsia con aguja fina bajo control ecográfico. Otras indicaciones de la laparoscopia fueron: corrección de falsos negativos en el estudio con biopsia dirigida, estadiaje pre-quirúrgico (en 15 de 157 pacientes) y, finalmente, control de la positividad de la punción-biopsia dirigida, en la fase inicial de utilización de éste método en nuestro Servicio.

En conclusión, puede decirse que la laparoscopia ya no es la técnica invasiva de elección en el diagnóstico histológico de los carcinomas hepatocelulares; sin embargo juega un papel importante, aún actualmente, en algunos casos seleccionados. La integración entre los resultados de la laparoscopia y de la biopsia dirigida por ecografía proporciona los mejores resultados diagnósticos.
