Secondary failure to sulphonylurea treatment

Summary

The incidence of secondary failure was 5.6 per cent, in 784 patients treated with sulphonylureas during a period of 7 years. Not all of those who relapsed needed insulin and half were successfully re-established with a combination of diguanide and sulphonylurea. Secondary failure sometimes becoming permanent may develop as the result of metabolic stress. Some of the factors contributing to the incidence of secondary failure are discussed, and it is concluded that poor primary selection and inadequate dietary adherence have contributed to the high incidence reported by some workers previously.
