Zusammenfassung
Grundlagen: In Zusammenhang mit „biologischen“ Osteosyntheseverfahren wurde ein anatomisch vorgeformter Fixateur interne für das distale Femur entwickelt, der in perkutaner Technik verwendet werden kann. Vorgestellt werden das Implantat, die Operationstechnik, die Komplikationen und Ergebnisse mit besonderer Berücksichtigung der Versorgung periprothetischer Frakturen.
Methodik: Im Zeitraum vom Juli 1997–Juli 1999 wurden an unserer Abteilung insgesamt 25 Patienten mittels LISS-DF osteosynthetisch versorgt (zunächst im Rahmen einer Mitbeteiligung an einer prospektiven Multicenterstudie).
Ergebnisse: Das Durchschnittsalter betrug 77 a (24–97 a), es handelte sich um 20 weibliche und 5 männliche Patienten. 10 Patienten erlitten eine periprothet. Fraktur resp. Fraktur bei liegendem Implantat (= 40% des Gesamtkollektivs), 17mal war das linke, 8mal das re. Femur betroffen. Die durchschnittliche Operationszeit betrug 146 Min. (Mehrfachosteosynthesen incl.), die durchschnittliche Strahlungszeit 4,7 Min. Insgesamt wurden, bezogen auf die AO-Klassifikation für das distale Femur, 7 A-, 3 B-, 3 C-Frakturen stabilisiert, 1 frakturgefährdete osteolyt. Metastase versorgt, und 1 supracondyläre Umstellungsosteotomie durchgeführt. Die periprothetischen Frakturen entsprachen Typ-3- und 4 Läsionen bzw. 5-A- und 5-B-Frakturen lt. AO. Der begleitende Weichteilschaden wurde nachTscherne klassifiziert, in unserem Patientengut wurde lediglich 1 zweitgradig offene Fraktur versorgt. Die Nachuntersuchungen erfolgten im wesentlichen gemäß des AO-Studienprotokolls, für ein längerfristiges Follow-up standen allerdings nur 16 Patienten (= 64%) zur Verfügung. Sekundäreingriffe waren bei 4 Patienten (= 17%) erforderlich, postoperative Infekte konnten wir nicht beobachten.
Schlußfolgerungen: Bei geeigneter Indikationsstellung und nach entsprechender Lernkurve stellt das LISS-DF System sicherlich eine gute Alternative gegenüber herkömmlichen Osteosyntheseverfahren dar. Insbesondere bei periprothetischen Frakturen und bei hochgradiger Osteoporose erscheint aber ein vom Standardverfahren abweichendes Vorgehen unter Verwendung additiver Implantate gerechtfertigt.
Summary
Background: According to “biological” osteosynthesis an anatomical preformed fixateur interne for distal femur fractures was developed. It may be used in percutaneus technique as well as by open procedure. We’ll show the implant, the surgical technique, the complications especially in treatment of periprosthetic fractures, and last but not least, the clinical results
Methods: From July 1997 to July 1999 in our departement 25 patients with distal or periprosthetic femoral fractures were operated by using LISS-DF. The first year we participitated in a prospective european multicenter study.
Results: The patients average age was 77 years (24–97y), there were 20 women and 5 men.10 patients (= 40%) suffered from periprosthetic femoral fractures. In 17 cases the left femur, in 8 cases the right femur was affected. 7 A, 3 B, 3 C fractures according to AO-scheme for distal femur fractures, 1 secondary bone tumor and 10 periprosthetic femoral shaft fractures were stabilized by surgery; 1 supracondylary osteotomy was carried out. The additional soft tissue injuries were classified by the Tscherne scheme the there were only 1 open fracture. The average time of surgery was 146 minutes (including double and triple osteosynthesis), the average c-arm counter time was 4,7 minutes. The clinical follow up investigations (about 6,9–12 weeks, 6 and 12 months after surgery) included until now only 16 patients (= 64%). In most cases those check up’s showed good functional results-we didn’t find any infection. In 4 cases further operations were necessary (2 plate loosenings, 2 primary displacements of plate or screws).
Conclusions: Depending on the type and the location of femoral fractures various treatment options are recommended. After managing the “learning curve” the results of LISS-DF technique are very good, though in case of periprosthetic fractures or serious osteoporosis additional osteosynthesis is recommended.
