Artroskopische Versorgung bei Impressions-Tibiakopffrakturen

Zusammenfassung

Grundlagen: Minimalinvasive Eingriffe sind in letzter Zeit bevorzugt gegen operativen Gelenkeröffnungen. Der Eingriff hat zwei Phasen. Der erste Teil ist eine Arthroskopie, gefolgt durch eine Reposition und eine Osteosynthese mit Schraube.

Methodik: 49 Patienten hatten eine arthroskopisch kontrollierte Impressionsaufhebung zwischen den Jahren 1994 und 1998.

Ergebnisse: Die Kniebewegungen der Patienten während der Kontrolle waren: sehr gut 18 (36%), gut 30 (62%), mäßig 1 (2%), schlecht 0.

Schlußfolgerungen: Die angewandte Operationstechnik bedeutet eine minimale Belastung den Patienten. Eine sorgfältige Kontrolle des lateralen Meniskus ist während der Operation notwendig. Spätarthrosen kommen selten vor wenn die Reposition exakt und die Schraubenfixation stabil ist.

Summary

Background: Operations with minimal invasives are frequently applied in cases of type AO B1-B2-B3 depressed fractures of the tibia. This management consists of an arthroscopy and a closed reposition followed by screw fixation.

Methods: 49 patients were treated by this technique between 1994 and 1998.

Results: Knee movements were excellent in 36% and good in 62% of all cases after one year. The procedure causes minimal operation stress to the patients.

Conclusions: The lateral menisci and condition of the joint cartilage should be carefully controlled during arthroscopy. Exact reposition and stable fixation result in low frequency of late arthroses.
