Zusammenfassung
Grundlagen: Tibiakopffrakturen sind ein ernsthaftes Problem. Stabilisation ist ein aufwendiges operatives Verfahren.
Methodik: In unserem Institut wurde eine eigene Platte entwickelt, die eine bessere Stabilisation und seitenverkehrte Anwendung möglich macht. Mit millimetergenauer Reposition und Spongiosa-Plastik wurden die Operationen seit 14 Jahren durchgeführt.
Ergebnisse: Die Spätresultate wurden nach der MethodeKellgren-Lawrence objektiv beurteilt. Die Röntgenbild-Analyse zeigte wenig Arthrose. Die subjektive Bewertung nachLequesne zeigte ebenfalls gute Ergebnisse. Bewegungsmöglichkeiten waren mäßig eingeschränkt, aber bei den meist älteren Patienten bedeutete das geringe Probleme.
Schlußfolgerungen: Die genaue Reposition ist bei den Gelenkfrakturen erforderlich. Es ist wichtig, um welchen Preis man das erreichen kann. Unserer Meinung nach ist die Invasivität ein zeitgebundener Begriff. Was heute minimal invasiv ist, wird morgen als viel zu aggressiv erscheinen.
Summary
Background: The knee is our largest joint. It is very vulnerable by the sport activity and by the RTA. Reconstruction there is a very important task for the trauma and emergency surgeons.
Methods: For the better results and the better stabilisation we introduced a new type of plate. This makes easier the stabilisation, and bilateral design provides better adaptation.
Results: After 14 years we made an X ray check up with 121 patients. The objective scoring byKellgren-Lawrence method showed a relative minimal occurrence of the OA. The subjective scoring was performed withLequesne method. 75% of the patients had the opinion that the results are good for their everyday life.
Conclusions: It seems to be adequate that the very correct reconstruction of the joint surface determines the future of the joint. To achieve this goal nowadays we try to be minimal invasive. The slogan minimal invasive is a time related entity. The surgeon has to be always optimal invasive, that means not more, but not less as it is necessary.
