Zusammenfassung
Grundlagen: Impressionsbrüche des lateralen Tibiakondyls sollten ab einer Stufe von 2 mm reponiert und übungsstabil osteosynthetisch versorgt werden. Durch minimal invasive Techniken werden einerseits die Weichteile geschont, andererseits kann die postoperative Morbidität gesenkt werden.
Methodik: Ein kortiko-spongiöser Knochenblock wird entnommen. Über dieses Fenster wird ein Zieldraht in das Zentrum der Impression vorgeschoben. Die Fraktur wird über einen kanülierten Stössel reponiert. Das Repositionsergebnis wird durch den gewonnenen Knochenblock unterfüttert und durch zwei kanülierte Schrauben gehalten. Eine Ruhigstellung ist für 4 Wochen erforderlich.
Ergebnisse: In den Jahren 1994–1997 wurden auf diese Weise 50 imprimierte Schienbeinkopfbrüche behandelt. Davon wurden 36 (17 Frauen, 19 Männer, Durchschnittsalter 53,2 Jahre) nachuntersucht.
35 von 36 Fällen waren subjektiv sehr zufrieden, hatten keine wesentliche Bewegungseinschränkung und hatten eine Gangleistung über 1 ½ Studen.
Eine Zunahme der vorbestehenden Arthrose fanden wir in zwei Fällen. In keinem Fall ergab sich eine Symptomatik des lateralen Meniskus. Ein Infekt trat nicht auf.
Schlußfolgerungen: Diese Methode kann aufgrund der geringen Weichteilschädigung und der guten Ergebnisse bei schonendem perkutanen Vorgehen bestens empfohlen werden.
Summary
Background: Impressions of the tibial plateau of more than 2 mm should be elevated and fixed with screws. The CPM therapy is necessary. The minimal invasive technique protects the soft tissues and has a tolerable complication rate.
Methods: A cortical bone block is prepared on the medial tibial cortex. A guide wire is placed under the deepest part of the impression. The fracture is reduced through a cannulated elevator. The resulting defect must be filled with the cortical bone block. Cannulated screws should be used to prevent secondary displacement. The range of motion may be increased gradually to 90° with the CPM machine. Then a cast for 4 weeks is necessary.
Results: We treated 50 fractures with this method between 1994 and 1997. 36 came to the follow-up examination. The mean age was 53,2 years, 17 were women and 19 were men.
35 of the 36 were happy with the result. They all had a sufficient range of motion and they could walk about 1 ½ hours. We saw a moderate increase of arthrotic deformity in 2 cases. We found no lesion of the lateral meniscus. There was no infection.
Conclusions: This technique should be used in plateau impressions because our follow-up shows very satisfactory results and no damage of the soft tissues.
