Standardisiertes operatives Vorgehen bei Tibiakopfimpressionsfrakturen

Zusammenfassung

Grundlagen: Plattenosteosynthesen bei Tibiakopfimpressionsfrakturen sind nach wie vor der Standard der operativen Versorgung.

Methodik: 45 Patienten mit lateralen Tibiakopfimpressionsfrakturen vom Typ B (AO) (37 Patienten) und Typ C (AO) (8 Patienten) wurden von uns 4–5 Jahre nach Trauma und osteosynthetischer Versorgung mit lateraler Tibiakopfabstützplatte und Unterfütterung des tibialen Defektes mit einem kortikospongiösen Span nachuntersucht. Weiterhin führten wir eine leichte Varisierung durch Überkorrektur der Fraktur durch.

Ergebnisse: Es fand sich eine hohe subjektive Zufriedenheit mit dem Langzeitergebnis. 94% der Patienten mit Frakturen vom Typ B hatten eine gute und seitengleiche Beweglichkeit im Kniegelenk. Bei den Patienten mit Typ-C-Verletzungen waren es jedoch nur 13%. Ähnliche Ergebnisse fanden sich bei der Arthroserate. 9,6% der Patienten mit Typ-B-Frakturen zeigten eine Arthrose und 37,5% der Patienten mit Typ-C-Frakturen. Ein vergleichbares Bild fand sich bei der Einnahme von Schmerzmedikamenten.

Schlußfolgerungen: Eine stabile Plattenosteosynthese am Tibiakopf und eine leichte Varisierung erlauben eine frühfunktionelle Nachbehandlung von Frakturen. Dadurch kommt es zu einer knorpelprotektiven Wirkung, worauf die guten subjektiven und objektiven Langzeitergebnisse zurückführen sind.

Summary

Background: The purpose of this investigation was to examine the long term results after plate osteosynthesis of tibial plateau fractures.

Methods: 4 to 5 years after accident 45 patients were examined. 37 of them had a Type B fracture (AO) and 8 of them had a Type C fracture. All of them had unicondylar displaced fractures that were repaired with a lateral plate and the consequently using of a bicortical splinter of the pelvis. The tibial plateau was overcorrected to get a small varisation of the knee.

Results: All patients examined were quite satisfied with the results after 4 to 5 years of operation. 94% of those with Type B fractures revealed the same motility in both knees, compared with the uninjured side. Patients with Type C fractures only had such results in 13%. Similar results were to be found concerning arthrosis rate. The patients examined with Type B fractures showed arthrosis of the knee in 9.6% compared to those with Type C fractures in 37.5%.

Conclusions: The technique of plate osteosynthesis in lateral tibial plateau fractures combined with a small varisation allows an anatomical repair and early rehabilitation with a good cartilage protection. Allthough Type C fractures show that more complex fracture patterns limit the results of long term investigations.
