Zusammenfassung
Grundlagen: 3 % bis 4 % aller Laparotomien erfolgen wegen intestinaler Obstruktion durch intraabdominelle Adhäsionen. Nach vorangegangener Laparotomie finden sich in über 90% Adhäsionen.
Methodik: In einer retrospektiven Analyse wurden 541 Patienten eingeschlossen, die von 1990 bis 97 wegen einer intestinalen Obstruktion operiert wurden. Bei 175 Patienten (32,4 %) fanden sich als Ursache Adhäsionen nach vorangegangener Laparotomie. Verglichen wurden die Gruppen mit früh-postoperativem Ileus (innerhalb von 4 Wochen; n=65) und spät-postoperativem Ileus (jenseits der 4 Wochen; n=110). Analysekriterien waren anamnestische, klinische und therapeutische Parameter. Die statistische Auswertung erfolgte mit dem „Fisher’s exact test“.
Ergebnisse: Zwischen beiden Ileusgruppen fanden sich keine Unterschiede bezüglich der klinischen Befunde. Beim spätpostoperativen Ileus war in 66,4 % der Fälle eine Bride (vs. 32,1 % früh-postoperativ, p<0,05) für den Verschluß verantwortlich. Demgegenüber bestand früh-postoperativ wesentlich häufiger eine Knickbildung am Darm (37,7 % vs. 17,3 % spätpostoperativ, p<0,05). Hinsichtlich der Ileuscharakteristika (akut / chronisch, komplett / inkomplett) bestanden keine Unterschiede. Therapeutisch war bei Patienten mit spät-postoperativem Ileus eine Darmresektion aufgrund ischämischer Darmnekrosen signifikant häufiger notwendig als bei Patienten mit früh-postoperativem Ileus (32 % vs. 17 %, p<0,05). Bei 51 % der Patienten mit früh-postoperativem und 17% der Patienten mit spät-postoperativem Ileus erfolgte die Einlage einer langen Dennissonde zur inneren Darmschienung.
Schlußfolgerungen: Beim adhäsionsbedingten Ileus nach vorangegangener Laparotomie sind abhängig vom Zeitpunkt des Auftretens die erwarteten Unterschiede in Bezug auf Verschlußursache und der daraus entstehenden Therapie zu erkennen.
Summary
Background: 3 % to 4 % of all laparotomies are performed for intestinal obstruction due to intraabdominal adhesions. In the case of relaparotomy adhesions are present in up to 90 %.
Methods: Between 1990 and 1997 541 patients were operated on because of intestinal obstruction. In 175 patients (32.4 %) the cause were adhesions after previous laparotomy. This retrospective study analyses patients who had the obstruction early (within four weeks; n=65) and late (n=110) after prior abdominal operation. Criteria were the history, physics and treatment. Statistical analysis was done with the Fisher’s exact test.
Results: There was no difference in the clinical presentation between both groups. Late-postoperatively a single adhesive band caused in 66.4 % the obstruction (vs. 32.1 % early-post-operatively, p<0.05). Kinking of the bowel was more often responsible for early-postoperative obstruction (37.7 % vs. 17.3 % late-postoperative; p<0.05). No differences were in the characteristics of the obstruction (acute / chronic, partial / complete). Bowel resection because of ischemic necrosis was significantly more often performed in patients with late-postoperative obstruction (32 % vs. 17 % early-postoperative; p<0.05). 51 % of patients with early-postoperative and 17 % with late-postoperative obstruction received a long intestinal tube.
Conclusions: In patients with adhesive intestinal obstruction we found dependent upon the time after prior laparotomy the expected difference in the causes of obstruction. This resulted in a different therapeutic approach.
