Zusammenfassung
Grundlagen: In der Abklärung nicht tastbarer Mammaherde steht seit April 1997 in Österreich das Advanced Breast Biopsy Instrumentation (ABBI®)-System zur Verfügung. Zur Überprüfung des Stellenwerts dieser neuen „minimal invasiven Operationsmethode“ wurde von der Österreichischen Krebshilfe landesweit eine Multizenterstudie initiert, über dessen erste Ergebnisse berichtet wird.
Methodik: ABBI besteht aus einer stereotaktischen Mammographieeinrichtung, welche mit einem speziell entwickelten Biopsieinstrument ausgestattet ist. Damit können mit großer Genauigkeit Gewebszylinder mit Durchmessern bis zu 2 cm aus dem Brustgewebe entnommen werden. Die Biopsie wird in Lokalanästhesie und ambulant durchgeführt. Bisher wurden 216 Patientinnen in die Studie aufgenommen. Es kamen 122 Mikrokalzifikationen, 79 Rundherde und 12 gemischte Veränderungen zur Abklärung.
Ergebnisse: Die ABBI-Biopsie konnte in 186 Fällen vollständig und ohne technische Komplikationen durchgeführt werden. In 24 Fällen konnte die Läsion nicht adäquat eingestellt und es mußte somit auf eine Biopsie verzichtet werden, in 6 Fällen konnte eine begonnene Prozedur wegen anderer Probleme nicht zu Ende geführt werden. Histologisch handelte es sich um 42 benigne Tumoren, 100 Mastopathien und andere benigne Veränderungen, 15 In-situ-Karzinome, 33 invasive Karzinome und andere maligne Veränderungen. Die Karzinome wurden überwiegend nachreseziert. ABBI ist bisher eine komplikationsarme Methode. In einer Patientenbefragung war ein überwiegender Teil (98,7%) mit dem Komfort bei der Operation zufrieden bis sehr zufrieden, der gleiche Prozentsatz an Patientinnen würde sich dem Eingriff wieder unterziehen.
Schlußfolgerungen: Abgesehen von den technischen Einschränkungen kann ABBI mit hoher Sicherheit verdächtige Herde vollständig aus der Brust entfernen und stellt aufgrund des Patientenkomforts eine sinnvolle Alternative in der operativen Abklärung nicht tastbarer Mammaherde dar. Die Frage über eine mögliche therapeutische Option beim Mammakarzinom bleibt noch weiter ungeklärt.
Summary
Background: The Advanced Breast Biopsy Instrumentation (ABBI®) System is available in Austria since April 1997. “Die Österreichische Krebshilfe“ — a national cancer society — has initiated a nationwide multicentre study to assess accuracy, safety and patient comfort of this new technique.
Methods: The ABBI-System allows radiologically guided stereotactic excision of non-palpable radiodense breast lesions. Tissue cylinders of 5, 10, 15 or 20 mm diameter and of variable lengths can be removed very accurately under local anaesthesia and on outpatient basis. 216 patients with clusters of microcalcifications (n=122), with round lesions (n=79) and of mixed type (n=12) were qualified for ABBI.
Results: 24 patients originally scheduled for the procedure were excluded, since the lesion could not be focussed adequately. The procedure had to be abandoned in 6 cases due to other reasons. 42 benign tumors, 100 cases of fibrocystic disease and other benign lesions, 15 in situ and 33 invasive malignancies were diagnosed. Most of the patients with malignancies had an “open” excision following the ABBI-procedure. Overall patient satisfaction was 98.7%, the same rate would undergo this technique again if necessary.
Conclusions: ABBI is a safe and highly accurate technique to totally excise non-palpable breast lesions if the lesion can be visualised adequately. For reasons of patient comfort it is a useful alternative to “open” biopsy. The discussion about ABBI being suitable not only just for diagnostic but also for therapeutic use has not been finished yet.
