Zusammenfassung
Grundlagen: Über die subjektive Lebensqualität nach Hernienoperation bei geriatrischen Patienten ist wenig bekannt. Die Indikationsstellung und die Operation selbst werden in dieser Altersgruppe sehr unterschiedlich gehandhabt.
Methodik: 90 über 75jährige Patienten wurden in Lokalanästhesie mittels anteriorer Patch-Plastik (Lichtenstein) operiert und 2–26 Monate postoperativ zur Lebensqualität und Zufriedenheit mit der Operation befragt.
Ergebnisse: Bei 83 Elektiveingriffen verlief die Operation in Lokalanästhesie problemlos. Probleme mit der Operation ergaben sich bei 7 Fällen mit Inkarzeration u.a. durch Fehleinschätzung der Situation. 97% der Patienten beurteilten das Operationsergebnis retrospektiv mit der bestmöglichen Note auf einer 5teiligen Skala. Bei 2 Patienten stellte sich als Ursache geringerer Zufriedenheit eine als mangelhaft empfundene postoperative Nachbetreuung heraus.
Schlußfolgerungen: Für ein subjektiv positiv empfundenes Ergebnis der Leistenhernienchirurgie im hohen Alter sind folgende Faktoren ausschlaggebend: zurückhaltende Indikationsstellung, Lokalanästhesie, optimale und schonende chirurgische Technik, persönliche perioperative Betreuung durch den Operateur.
Summary
Background: Little is known about the subjective quality of life after inguinal herniorrhaphy in elderly patients. There is wide variance for indications and operative techniques in that age group.
Methods: 90 patients older than 75 were operated under local anesthesia using the anterior (Lichtenstein) patch plasty. Patients were interviewed 2–26 months postoperatively about quality of life and satisfaction with surgery.
Results: No operative problems were encountered in 83 elective interventions under local anesthesia. In seven emergency operations problems were due partly to faulty preoperative evaluation. 97% of patients rated the operative result with best satisfaction of a 5-step analogue rating scale. Less satisfaction was attributed in two patients to subjectively resented suboptimal perioperative care.
Conclusions: The following factors were identified for optimal subjective result of inguinal hernia repair in the elderly: restrictive indication, local anesthesia, optimal and subtle surgical technique, personal perioperative care by the responsible consultant.
