Zusammenfassung
Grundlagen: Eine häufige Ursache des chronischen ulnocarpalen Handgelenksschmerzes ist das Ulnar-Impaction-Syndrom.
Methodik: Beim ulnocarpalen Impingement handelt es sich um eine chronische, axiale Drucküberlastung durch Einklemmung des ulnocarpalen Bandkomplexes (TFCC) zwischen dem Ulnakopf auf der einen Seite, dem Triquetrum und Lunatum auf der anderen Seite.
Ergebnisse: Die Druckverhältnisse im ulnocarpalen Gelenkkompartment ändern sich bereits durch geringfügige Verschiebungen des Längenverhältnisses zwischen Ulna und Radius. Neben der konservativen Therapie stehen bei der Behandlung Eingriffe am TFCC, offen oder arthroskopisch, die Ulnaverkürzungsosteotomie und bei Beteiligung und Inkongruenz des distalen Radioulnargelenkes die Operation nachKapandji, die Ulnaköpfchenresektion oder eine ihrer zahlreichen Varianten zur Verfügung. Auch die sogenannte Wafer-Operation stellt eine mögliche Option dar.
Schlußfolgerungen: Die Ulnaverkürzungsosteotomie stellt eine wirkungsvolle Therapie bei anlagebedingten wie auch posttraumatischen Plus-Varianten dar, wobei eine ausreichende Stellung der Radiuskonsole erhalten sein muß.
Summary
Background: Ulnar impaction syndrome is one of the most frequent reasons for chronic ulno-carpal wrist pain.
Methods: Ulno-carpal impingement is caused by chronic axial overload due to impaction of the Triangular Fibrocartilage Complex (TFCC) between the ulnar head and the proximal carpal row (triquetrum and lunate bone).
Results: Intracompartmental ulnocarpal pressure is altered by only minimal changes of the length relation between ulna and radius. Treatment options are either conservative or operative. Surgical techniques include revision of the TFCC (either open or arthroscopic), ulna, shortening osteotomy and in case of DRUI — incongruicy Kapandji-procedure, ulnar head resection and modifications or Wafer’s procedure.
Conclusions: Ulnar shortening osteotomy is a reliable surgical method in cases of congenital method in cases of congenital or posttraumatic ulna-plus-variants. However, good position of the distal radial console is an important predisposition of this technique.
