Die chirurgische Technik der Fasziotomie an der oberen und unteren Extremität

Zusammenfassung

Grundlagen: Die Fasziotomie stellt eine Notfallsmaßnahme bei Patienten mit akutem Kompartmentsyndrom der oberen oder unteren Extremität dar.

Methodik: In einer Übersicht wird die chirurgische Technik der Fasziotomie an der oberen und unteren Extremität beschrieben.

Ergebnisse: Neben der frühzeitigen Diagnostik und Notfallsoperation stellt die ausreichende Entlastung aller betroffenen Kompartments die Voraussetzung für den normalen Funktionserhalt der Extremität dar. Die chirurgische Technik orientiert sich an der Anatomie der einzelnen Kompartments unter Berücksichtigung der Gefäßnervenstraßen.

Schlußfolgerungen: Bei rechtzeitiger Indikation und exakter chirurgischer Technik können auch bei Patienten mit akutem Kompartmentsyndrom gute funktionelle Ergebnisse erzielt werden.

Summary

Background: Fasciotomy is an emergency surgical procedure in cases of acute compartment syndrome of the upper and lower limb.

Methods: This review article describes the surgical technique of fasciotomy of the upper and lower limb.

Results: Both early diagnosis, emergency surgery and sufficient decompression of all compartments are important requirements for the functional rehabilitation of the injured limbs. Surgical procedures are determined by the anatomic situation of the singular compartments, especially focusing on the course of the neuro-vascular bundles.

Conclusions: Early diagnosis and correct surgical procedures are the basis for good functional outcome in patients with severe acute compartment syndromes.
