Electrocardiographic changes produced by septal rupture after acute myocardial infarction

Summary

Electrocardiographic changes produced by septal rupture following acute myocardial infarction in 4 patients are described. All 4 patients had anterior wall myocardial infarction and in 1 there was lateral extension. The electrocardiographic changes at the time of septal rupture included marked rise in ST segment and supraventricular tachyarrhythmias. P wave configuration changes, which suggested ventricular overload were present in all patients. The mean frontal QRS axis shift occurred in all patients between 1 to 35 days from the onset of septal rupture. Mean QRS axis shift occurring early was associated with poor prognosis. There was no correlation between the sum of ST segment elevation in the standard six precredial leads (Σ ST0) and the size of the shunt or the defect.
