Zwerchfellrupturen im Kindesalter

Zusammenfassung

Obwohl insgesamt seilen, haben wir in den letzten Jahren die kindliche Zwerchfellruptur zunehmend häufiger beobachtet. Vier eigene Fälle werden beschrieben, und mit der Literatur verglichen. Zwerchfellrupturen im Kindesalter sind meist linksseitig, aber in einem Drittel der Fälle fand sich eine Verletzung des rechten Zwerchfelles. Im Gegensatz zum Erwachsenen sind Rippenfrakturen beim Kind eine seltene Begleitverletzung. Häufig sind Leber und Milz verletzt, und es kommt zum Prolaps von Abdominalorganen in den Thorax. Die Prognose der isolierten Zwerchfellverletzung ist gut, jedoch wird sie in der Regel durch die schweren Begleitver-letzungen bestimmt. Nur in der Hälfte der Fälle wurde die Zwerchfellruptur initial erkannt.

Abstract

Although diaphragmatic rupture in children due to blunt trauma is a rare condition we have observed an increasing number of these cases during the last years. Four cases will be reported and compared to the literature. The majority of diaphragmaticruptures occurred on the left side. One third of all patients, however, had a rupture of the right diaphragm. In contrast to adults rupture of the diaphragm in children is rarely associated with rib fractures. Frequently liver and spleen become injured and two thirds of the patients presented vvith prolaps of abdominal organs into the pleural cavity. The prognosis of isolated diaphragmatic rupture is good, usually, however, prognosis is limited by the severe injuries sustained by the initial trauma. Only in half of the cases diaphragmatic rupture was diagnosed initially.
