Remarks on surgical therapy of the early gastric carcinoma viewed from analysis of its growth patterns

Conclusion

Surgical therapy of the early gastric carcinoma were discussed from the view point of a new knowledge of its growth patterns and summarized as follows:

  1. (1)

    Proper cutting line of the stomach at the time of gastrectomy may be determined 2 cm. apart from the margin of the macroscopic lesion.

  2. (2)

    Overall five and ten year survival rate of the early gastric carcinoma was 93.3% and 88.6% respectively. However, it must be pointed out that there exsists relatively few but definite incidence of type of poor prognosis, and we successfully separated such a form as “penetrating form” from the usual superficial spreading type of carcinoma. Five year survival rate of the latter was 100%, while that of the former was so poor as 50%.

  3. (3)

    Intramucosal carcinoma and widely superficial extended type of carcinoma show favorable prognosis. However, care must be taken to the small but penetrating type of carcinoma. In the latter type, superradical enucleation of the lymph node metastases as well as adjuvant anticancer chemotherapy are recommended at the time of gastrectomy.
