Biochemistry of protein-calorie malnutrition a review of some recent advances

Conclusion

Work on the biochemical aspects of P.C.M. recently has added to our knowledge of its pathophysiology. The metabolic derangements quickly become apparent in kwashiorkor and may lead to gross pathological abnormalities such as oedema and fatty liver. This is due to failure or adaptation, possibly because of the inhibitory effects of dietary carbohydrate on tissue catabolic processes. The general absence of metabolic abnormalities and cellular derangements in marasmas is indicative of the ability to maintain homeostasis for prolonged periods in the virtual absence of food.

Observations such as altered intestinal enzyme activities, lactose intolerance, diminished xylose absorption, presence of unconjugated bile salts and reduced immunity are some of the aetiological factors for the causation of diarrhoea in-P.C.M.

Some of the significant biochemical parameters for detection of sub-clinical P.C.M. and for following the course of the disease are: plasma albumin, TBPA,. ratio of essential to nonessential aminoacids, hydroxyproline index, pseudocholinesterase activity, muscle potassium and urinary creatinine.
