Prevention of recurrent infection after removal of intramedullary metal of the infected femur and tibia

Summary

Prevention of recurrence after consolidation of infected femur and tibia fractures and nonunions is based on two principles. First necrotic bone is eliminated with reaming and rinsing the intramedullary canal after metal removal. As missing inramedullary tissue granulates gradually, prevention of retention cavities is achieved by a drainage tube that is gradually removed in the direction of gravity. In chronic cases the method is combined with application of a Septopal chain. Since 1978 this method has been used in 39 cases of which 32 have been succesful.

Résumé

Les auteurs estiment qu’après la consolidation des fractures fémorales, tibiales et des pseudarthroses infectées, la prévention de l’évolution des récidives est basée sur deux principes :

  1. -

    Lors de l’ablation du clou centromédullaire, la nécrose osseuse doit être éliminée par alésage et rinçage de la cavité médullaire.

  2. -

    Il convient d’empêcher la création de cavités rétentives de tissus de granulation par ablation progressive d’un tube de drainage dans le sens de la gravitation.

Dans les cas chroniques, les auteurs y adjoignent l’utilisation de la chaine Septopal. Ce procédé thérapeutique est utilisé depuis 1978, sur 39 cas 32 ont donné entière satisfaction.
