Increase in the incidence of non-Hodgkin’s lymphoma, clinical features and long-term follow-up

Aumento de la incidencia del linfoma no Hodgkin, características clínicas y estudio de factores pronósticos tras un largo seguimiento

Abstract

We detail time trends in the incidence of non-Hodgin’s lymphoma over the period 1980–1894 and analyze prognostic factors in a retrospective study.

To that end, 587 patients were diagnosed of non-Hogdkin’s lymphoma from 1980 to 1994.

Average annual incidence rate of non-Hodgkin’s lymphoma per 100,000 persons in our study has risen from 5.5 in 1981–1985 to 8.6 in 1991–1994. The incidence rate increased by almost 22% in males, and by 79% in females. In patients 75 years and older the incidence has increased 98%. The largest increases of incidence were observed in extranodal involvement (86%), stage IV (77%) and intermediate grade (53%). Prognostic factors for the overall survival in the multivariate analysis were: histology (p<9.00001), International Prognostic Index (p<0.00001), largest site localization (p=0.0002) and mediastinal involvement (p=0.003).

We conclude that to the known prognostic factors as histology and International Prognostic Index, mediastinal involvement and the largest site remain independently significant in multivariate analysis. There was an overall increase in incidence of non-Hodgkin’s lymphoma. It was slightly more marked in patients older than 74 years, women, intermediate-grade lymphoma, stage IV and extranodal disease. Explanations accounting for all the increases in rates are not readily available.

Resumen

Se analiza la incidencia del linfoma no Hodgkin (LNH) durante el período 1980–1984, así como los factores pronósticos en un estudio retrospectivo.

Entre 1980 y 1994 se diagnosticaron 587 pacientes de LNH.

La tasa de incidencia media anual del LNH por 100.000 habitantes en nuestro estudio se ha incrementado desde 5,5 durante el período 1981–1985 hasta 8,6 en el período 1991–1994. En los varones la incidencia se ha incrementado en un 22% y en las mujeres en un 7%. En los pacientes de 75 años o mayores la incidencia se ha incrementado en un 98%. El mayor aumento de la incidencia se observó en la afectación extraganglionar (86%), estadio IV (77%) y la histología de grado intermedio (53%). Los factores pronósticos para la supervivencia global en el análisis multivariado fueron: la histología (p<0,00001), el índice pronóstico internacional (IPI) (p<0,00001), la localización de mayor volumen (p=0,0002) y la afectación mediastínica (p=0,003).

En conclusión, a los factores pronósticos ya conocidos como la histología y el IPI se añaden la afectación mediastínica y la localización de mayor volumen y permanecen con valor pronóstico independiente en el análisis multivariado. Se observó un incremento global en la incidencia del LNH. Fue más marcado en los pacientes mayores de 74 años, mjeres, grado intermedio, estadio IV y afectación extraganglionar. Actualmente no se conocen las causas del incremento de los LNH en los últimos años.
