Zusammenfassung
Es wurden 200 Aneurysmen aus den Jahren 1983 bis 1985 retrospektiv überprüft. Für diese Untersuchung wurden Computertomographie (CT), Isotopenzisternographie und operative Ergebnisse herangezogen.
Mittels CT fand sich bei fast 20% der Patienten Blut in den Hirnkammern: bei Karotisaneurysmen bei 11% und bei Aneurysmen der A. cerebri anterior bei 23%, weiters bei Aneurysmen der A. cerebri media bei 15%, und hier—mit einer Ausnahme—immer mit einem intrazerebralen Hämatom vergesellschaftet. Die vertebrobasilären Aneurysmen zeigten bei 55% Blutungen in die Hirnkammern, aber nie ins Parenchym. Der blutige Liquor im Ventrikelsystem bei Patienten, bei denen kein Ventrikeleinbruch dokumentiert werden konnte, legt den Schluß nahe, daß im Akutstadium der Aneurysmenblutung ein subarachnoidaler Blutreflux von den basalen Zisternen in das Ventrikelsystem besteht.
An Hand der Isotopenzisternographie konnten wir diese Theorie eindrucksvoll stützen.
Summary
In a retrospective analysis 200 aneurysms from 1983 till 1985 were examined. For this study, computer-assisted-tomography (CT), radioisotope-cisternography and operation results were considered. By means of CT in 20% of the patients blood was found in the ventricles: in aneurysms of the A. carotis in 11%, in aneurysms of the A. cerebri anterior in 23% and in patients with aneurysms of the A. cerebri media in 15%. Aneurysms of the vertebro-basilar arteries revealed hemorrhage in the ventricular system at a rate of 55.
Intraventricular hemorrhage without visible penetration into the ventricle is caused by acute blockade of cerebrospinal fluid circulation in the basal cisterns.
This reflux could be fully demonstrated by radioisotope-cisternography.
