Zusammenfassung
Um die Bedeutung der adjuvanten Therapie des Kolonkarzinoms zu beleuchten, müßten wir auf die gegenwärtige Situation des Kolonkarzinoms eingehen. In den USA erkranken jährlich etwa 110.000 am Kolonkarzinom (Inzidenz 34), in Österreich 1600.50% der Patienten sterben an ihrer Grunderkrankung. Die Prognose ist engstens verknüpft mit dem Tumorstadium zum Zeitpunkt der Diagnosestellung. Aufgrund positiver Ergebnisse entsprechender adjuvanter Therapiestudien bei Kolonkarzinom im Stadium III mit 5-Fluorouracil (5-FU) und Levamisol hat eine Consensus-Konferenz am National Cancer Institute der National Institutes of Health (NIH) in Bethesda im April 1990 die adjuvante Therapie mit 5-FU und Levamisol als “Standardtherapie” außerhalb von kontrollierten Studien empfohlen, wenngleich auch festgehalten wurde, daß zur Zeit keine optimale Therapieform existiert, so daß weitere Therapiestudien zu fordern sind und möglichst viele Patienten innerhalb dieser behandelt werden sollten.
Summary
Colon cancer is a major public health problem everywhere in the world. The annual incidence of colon cancer in the United States is approximately 110.000 new cases (34 cases per 100.000), in Austria 1600 cases. 50% of the patients with colon cancer will die, and the outcome of the disease is depending on tumor stage. New data from several studies have demonstrated statistically significant results in stage III colon cancer patients. Based on these results the National Institutes of Health Consensus Development Conference in Bethesda, Maryland, in April 1990 recommended, that patients with stage III colon cander should receive adjuvant therapy with 5-fluorouracil (5-FU) and levamisole outside controlled clinical studies, but because no optimal form of adjuvant therapy yet exists there is still need for further trials.
