Zusammenfassung
Zwischen 1980 und 1990 wurden 471 Thrombendarteriektomien der A. carotis interna an 437 Patienten durchgeführt. Kriterien zur Indikationsstellung waren: 1. hämodynamisch relevante Karotisstenose oder embolisierende Plaques in kausaler Beziehung mit der Klinik, 2. Ausschluß von schweren Begleiterkrankungen, intrazerebralen Blutungen und frischen Apoplexien. Es wurde in Allgemeinanästhesie ohne Shuntapplikation operiert.
Die perioperative Letalität aus neurologischer Ursache betrug 2% und konnte in der Zeit 1985 bis 1990 (in dieser Periode wurden nur Patienten im Stadium I und II operiert) auf 1,4% reduziert werden; die permanente neurologische Defizitrate betrug 0,7% in dieser Periode.
Ein Risikoprofil für neurologische Komplikationen wurde retrospektiv erarbeitet: Blutdruckabfall während der Karotisklemmung und lange Klemmphasen korrelierten mit schlechten Ergebnissen. Der Grad der kontralateralen Stenose, Shuntverwendung oder der Stumpfdruck hatten keinen Einfluß auf das operative Ergebnis. Die Karotisthrombendarteriektomie ist die Therapie der Wahl beim Patienten im Stadium II. Die indikation zur Operation beim asymptomatischen Patienten (Stadium I) wird heute restriktiv gesehen; prospektive Studien sollen klären, welche asymptomatischen Patienten eventuell doch von einer Karotisoperation profitieren. Im Stadium III und IV wird heute nicht mehr chirurgisch interveniert.
Summary
471 carotid endarterectomies were performed in 437 patients in the period from 1980 to 1990. Indication for surgery was based on the following criteria: hemodynamic efficacy of the stenosis or risk of embolism from a vascular lesion; exclusion of a severe concomitant disease which could augment the operative risk, exclusion of an intracerebral hemorrhage or a recent stroke.
The operation was performed in general anesthesia and without shunt. Perioperative mortality due to neurologic complications amounted 2%. Between 1985 and 1990 (stage I and stage II patients only) mortality could be reduced to 1.4%, a permanent neurological deficit occurred in 0.7% during the last 5-years-period.
Retrospectively high neurologic risk patients could be identified: dropping blood pressure during carotid clamping and long clamping phases can be correlated with poor results. No correlations could be obtained regarding the degree of the contralateral stenosis, the application of intraluminal shunts or the stump-pressure. Surgical intervention is regarded as the method of choice in stage II patients. Carotid surgery for stage I is seen restrictively and can not be recommended at present; prospective studies investigating the indication for asymptomatic patients have been started. Stage III and Stage IV (per se) is not operated on.
