Zusammenfassung
In über 10% ist in der Nachbarschaft von Halswirbelluxationen oder Luxationsfrakturen mit zusätzlichen Verletzungen von Bewegungssegmenten zu rechnen. Diese sind auf den herkömmlichen Aufnahmen schwer zu erkennen, fallen jedoch durch ein Klaffen unter Längszug auf. Da bislang Normwerte für das Ausmaß der Distraktion fehlen, wurde an Leichen sowie isolierten Halswirbelsäulen und einzelnen Bewegungssegmenten die Distanzzunahme der Halswirbel unter steigender Extension bis 30 kp gemessen: Gesunde Segmente lassen sich nur geringfügig (Maximalwert 1,4 mm) distrahieren, weshalb jeder höhere Wert Hinweis für eine traumatische Gefügelockerung ist. Die höchste Dehnbarkeit zeigte C4/5, gefolgt von C5/6 und C3/4, während C6/7 und C7/Th1 nur eine minimale Dehnbarkeit aufweisen.
Summary
Additional injuries to motion segments must be suspected in more than 10% adjacent to cervical spine dislocations or fractures. These hidden injuries often are not detected on conventional films, but are obvious by some gaping under longitudinal traction. Since there are no standard values for the amount of distraction, the distances of the cervical vertebrate were measured post mortem under increasing traction up to 30 kp: normal motion segments revealed only little distraction values (maximum value 1.4 mm). Distances of more than that are highly suspicious of traumatic instabilities. The highest compliance was found in C4/5, followed by C5/6 and C3/4, whereas C6/7 and C7/Th1 showed only a minimal compliance.
