Zusammenfassung
In dieser studie untersuchten wir veränderungen im wasser- und fettgehalt sowie in den freien intrazellulären aminosäurekonzentrationen des muskelgewebes bei patienten mit unterschiedlichen lebererkrankungen wie leberzirrhose, coma hepaticum, fulminantes leberversagen und lebertumoren. Bei allen patientengruppen waren sowohl der fettgehalt als auch der Wassergehalt des muskels hõher als bei gesunden erwachsenen. Der intrazelluläre wassergehalt, bestimmt mit der chloridmethode, war bei allen patientengruppen erniedrigt, der extrazelluläre wasseranteil erhõht. Patienten mit fulminantem leberversagen hatten im muskelgewebe wie im plasma durchwegs erhõhte aminosäurekonzentrationen, wobei diese Veränderungen im muskel weniger stark ausgeprägt waren. Die intrazhellulär erhõhten glutamin-und erniedrigten glutamatspiegel lassen auf eine muskuläre ammoniakentgiftung via glutamat-glutamin rückschließen.
Summary
We investigated metabolic alterations in muscle tissue of patients with liver diseases (carcinoma, cirrhosis, hepatic coma, fulminant hepatitis). In all four patient groups fat and total water content and also the proportion of extracellular to intracellular water was higher than in healthy volunteers. Patients with fulminant hepatitis had increased levels of nearly all free muscular amino acids (exception: asparatate, glutamate, citrulline). The increased proportion of intracellular free glutamine to glutamate indicates a muscular ammonia detoxification via this pathway.
