The size of the bile duct following cholecystectomy and choledochotomy: Pre-and postoperative determination in 101 patients by sonography

Zusammenfassung

Prä- und postoperativ wurde mit ultraschall die weite des ductus hepatocholedochus bei patienten mit cholezystektomie (CHE) (n=84) sowie mit CHE und choledochotomie wegen gallengangssteinen (n=17) verglichen. Dabei zeigt sich, daß nach einer einfachen CHE in über 70% keine, in 30% hingegen eine mäßige erweiterung (bis maximal 10 mm) auftreten kann. Die mittelwerte der prä- und postoperativen gallengangsweite sind 5,4 mm bzw. 5,5 mm (kein signifikanter unterschied).

Bei der gruppe mit zusätzlichem eingriff am choledochus kommt es zu einer vom ausgangswert abhängigen weitenabnahme des gangkalibers, die durchschnittlich etwa 50% beträgt. Der mittelwert der postoperativen weite ist hochsignifikant kleiner im vergleich zum präoperativen mittelwert.

Summary

Postcholecystectomy dilatation of the bile duct has been a matter of controversy in the radiographic literature. In this study, 84 patients had biliary sonography before and after cholecystectomy. More than 70% showed no evidence of postcholecystectomy dilatation of the bile duct. There was an enlargement up to 10 mm in 30% of patients. No significant difference between the mean diameter of the common duct before and after cholecystectomy was observed.

17 patients underwent cholecystectomy with bile duct exploration because of choledocholithiasis. All of them showed a postoperative diminution of the bile duct. There was a significant difference between the pre- and post-operative bile duct diameter.
