Zusammenfassung
Wenige Stunden nach einem stumpfen Thoraxtrauma entwickelte sich bei einem jugendlichen Patienten ein letal verlaufender Vorderwandinfarkt. Die Obduktion ergab als Ursache der Infarzierung ein ausgedehntes Wandhämatom im Anfangsteil des Ramus interventricularis anterior, das offenbar zu einer kritischen Beeinträchtigung der Koronarzirkulation geführt hat. Die Kasuistik zeigt, daß sich auch bei Jugendlichen ohne präexistente Koronarsklerose nach stumpfen Traumen ein Myokardinfarkt infolge einer gestōrten Koronarperfusion entwickeln kann.
Summary
Few hours after a blunt chest trauma a lethal acute anterior wall infarction occured in a juvenile man. The cause, a large haematoma in the first third of the LAD, evidently causing a critical obstruction of the vessel, was revealed by the post-mortem examination. As this case report shows a myocardial infarction after a blunt chest trauma due to a wall haematoma of a cononary vessel can even occur in juveniles without pre-existent coronary artery sclerosis.
