Ergebnisse der Gefäßchirurgie in einem Schwerpunktkrankenhaus

Zusammenfassung

Bericht über die gefäßchirurgische Tätigkeit an einem Schwerpunktrankenhaus, wobei Wert auf Rentabilität der therapeutischen Handlungen und Miteinbeziehung sämtlicher aggressiver angiologischer Behandlungsformen (Lyse, Angioplastik) gelegt wird. In einem Zeitraum von 18 Monaten wurden 106 gefäßrekonstruktive Eingriffe (davon 4 Aneurysmaresektionen, 27 Karotisendarteriektomien) durchgeführt; Letalität 2,8%, Amputationsrate 4%, wobei 85% der Patienten sich im Stadium III oder IV befanden. Nach 20-2 Monaten leben 83% der Patienten mit peripherer arterieller Verschlußkrankheit mit offener Rekonstruktion. Infolge der geringen Revisionsrate (8% Sofortverschluß, 2 Nachblutungen), des relativ geringen Operationsrisikos und der annehmbaren Spätergebnisse wird die Durchführung gefäßrekonstruktiver Operationen im Schwerpunktkrankenhaus für sinnvoll erachtet.

Summary

The possibilities and results of vascular surgery in a municipal hospital (872 beds, 8 operative departments) in the city of Vienna (Austria) are reported: Within a period of 18 months 106 operations (25 aorto-femoral, 50 femoro-popliteo-crural reconstructions, 4 aortic aneurysms, 27 carotid endarterectomies) were performed: early failure in lower limb reconstructions 8% (rate of amputation 4%), operative mortality 2.8%. Only 15% of the patients had claudication, 85% rest pain or gangrene. Because of the low number of reinterventions (7,5% postoperative rethrombosis or hemorrhage), the 2.8% mortality and the reasonable 20-2 months' follow-up results (83% good function) we believe that arterial reconstructive surgery has a place in the field of operative activities of a municipal hospital. To rationalize vascular surgery under these condition (high costs, long operating time) all patients with adequate lesions are transferred to streptokinase therapy and transluminal angioplasty.
