Läsionen des Nervus opticus und Chiasmas nach gedecktem Schädelhirntrauma

Zusammenfassung

Bei 1462 Patienten mit schwerem gedecktem Schädelhirntrauma wurden 21 mit traumatischen Läsionen des N. opticus und Chiasmas gesehen. Bei sofortigem Sehverlust erwiesen sich chirurgische Eingriffe als nutzlos. Einengungen des Canalis opticus durch frakturierte Knochensegmente erscheinen für die operative Intervention geeignet, doch waren die Ergebnisse enttäuschend. Diagnose, operatives Vorgehen und Prognose dieser Verletzungen werden disktiert.

Summary

Reviewing 1462 patients with head injury 21 cases of traumatic lesions of the optic nerve and optic chiasma have been collected. We were impressed by the following facts. If visual loss due to optic nerve injury is immediate, surgery never helps. Narrowing of the optic nerve channel due to fractured bone segments needs surgery, the results however, are not rewarding. Exploration, diagnosis and outcome of this injury is discussed.
