Zusammenfassung
Der Anteil nicht palpabler Mammatumore nimmt durch die Verbesserung, der diagnostischen Hilfsmittel ständig zu. Auf diagnostischem Gebiet besteht bei diesen Tumoren das Problem der exakten intraoperativen Lokalisierung, das nur in enger Zusammenarbeit von Chirurgen und Röntgenologen zu lösen ist. Während bei den invasiven Kleinstkarzinomen die brusterhaltenden Operationsverfahren erfolgversprechende Resultate zeitigten, die jedoch noch durch Langzeitresultate bestätigt werden müssen, ist die adäquate Therapie der präinvasiven Karzinome (IDC, CLIS) noch strittig. Beim IDC wird wegen der Gefahr der Multizentrizität und Bilateralität und dem Risiko zurückgelassener Tumorherde eine Therapie wei bei den invasiven Kleinstkarzinomen empfohlen. Beim CLIS bestehen wegen des hohen Risikos eines nachfolgenden ipse-oder kontralateralen Karzinoms große Differenzen über die ideale Therapie, die von regelmäßiger Kontrolle nach bioptischer Diagnosestellung bis zur bilateralen Mastektomie, mit sofortiger plastischer Brustrekonstruktion reichen. Die fibrozystische Mastopathie-der häufigste Befund bei den nicht palpablen Mammatumoren-ist bei Vorliegen höhergradiger Zellatypien mit einem erhöhten Mammakarzinomirisiko behaftet, so daß diese Patientinnen zu regelmäßigen Kontrollen einberufen werden sollten.
Summary
The frequency of non-palpable mamma tumours is steadily increasing, as the accuracy of the diagnostic aids is improving. The diagnostic problem in these tumours is the correct localization of the tumour intraoperatively. This can only be achieved by an intimate cooperation between surgeon and radiologist. In minimal breast cancer there have been excellent results with breast preserving operation techniques, which however have to be confirmed by long term follow up studies. The adequate therapy of preinvasive carcinomas, (IDC, CLIS) is still controversial. In the case of IDC there is the same treatment recommended as in the minimal breast cancer group, because of the bilaterality and multicentricity and the high incidence of residual tumors after biopsy. The ideal therapy of CLIS is still under discussion, as the risk of subsequent carcinoma is very high. The therapy ranges from simple biopsy with periodical follow up to a bilateral mastectomy with simultaneous breast reconstruction. The fibrocystic mastopathy, being the most frequent finding in non-palpable mamma tumours, is associated with an increased risk of mamma carcinoma in the presence of high grade cell atypia. Thus these patients should be called in for periodical controlls.
