Die mehrfachen Verletzungen des Beckenringes

Zusammenfassung

Im Krankengut der Unfallchirurgischen Klinik des Zentrums für Chirurgie in Gießen kamen unter 5515 stationär behandelten Unfallpatienten 168 Beckenfrakturen (3%) vor. Nur in 0,1% aller stationär wegen Knochenbrüchen behandelten Patienten lagen doppelte Verletzungen des Bekkenringes vor. Diese schwersten Traumatisierungen des Beckenringes lassen sich in vier Grundtypen einteilen, nämlich die Luxation im Bereich der Symphyse und gleichzeitige Luxation des Sakroiliakalgelenkes, den Vertikalbruch durch Schambein und Sitzbein unter gleichzeitiger und gleichseitiger Luxation des Sakroiliakalgelenkes, die Ruptur der Symphyse und gleichzeitige und gleichseitige aber gegebenenfalls auch doppelseitige Fraktur des Darmbeines oder Kreuzbeines paramedial oder paralateral des Sakroiliakalgelenkes und schließlich die doppelte Vertikalfraktur des vorderen und hinteren Beckenringes. Entstehungsmodus, Indikation zur konservativen oder operativen Behandlung werden anhand von 6 Beispielen besprochen.

Summary

Out of 5515 ward patients treated for fractures in the traumatological clinic of the surgical hospital in Gießen 169 (3%) had fractures of the pelvis. Only in 0,1% of all ward patients treated after accidents was a double fracture of the pelvic ring observed. These, by far the severest traumas in the pelvic ring can be classified in four fundamental types namely, the dislocation in the symphysis combined with dislocation of the sacro-iliacal joint, the ventral fracture through the pubis and ischium combined with the dislocation of the sacroiliac joint on the same side, the rupture of the symphysis combined with fracture on the same side but occasionally also with fractures on both sides of the ileum or of the sacrum paramedial or paralateral to the sacro-iliac joint and lastly the combined double vertical fracture of the anterior or posterior pelvic ring. Sic cases were presented and the mode of origin and indication for conservative or operative treatment were discussed.

Résumé

Sur tout le lot de malades hébergés dans la clinique du Centre Chirurgical de Gießen se sont révélées 168 fractures du bassin pour 5515 victimes d’accidents sous traitement stationnaire. Seulement 0,1% de l’ensemble des patients traités pour fracture du bassin présentait de doubles lésions de l’anneau pelvien. Ces lésions de l’anneau pelvien se classent en 4 types fondamentaux qui sont: luxation au niveau de la symphyse et luxation coïncidente de l’articulation sacriliaque, cèle verticale en travers du pubis et de la tubérosité ischiatique sous luxation coïncidente et homolatérale de l’articulation sacriliaque, rupture de la symphyse et fracture coïncidente et homolatérale, mais en plus, dans ce cas, des deux côtes de l’ilium ou sacrum paramédialement ou paralatéralement de l’articulation sacroiliaque, et enfin double fracture verticale de l’anneau pelvien. Le mode de genèse et les indications pour un traitement par opération ou par conservation sont commentés à l’appui de 6 exemples.
