Zusammenfassung
Gegenüber der unbefriedigenden konservativen Therapie verspricht die Ostesynthese verschobener Acetabulumfrakturen in solchen Fällen verbesserte Behandlungsergebnisse, in denen Subluxation und Gelenkinkongruenzen alleinige Ursache funktioneller und degenerativer Gelenkschäden sind. Darüber hinaus beeinträchtigt eine Reihe allgemeiner und lokaler Faktoren schicksalhaft das Behandlungsergebnis. Wahl des Zuganges und operative Taktik sind entscheidend für den Erfolg der schwierigen und aufwendigen Rekonstruktionen. Sie setzen die Kenntnis des Frakturverlaufes und der Frakturtypen voraus und erfordern neben den Röntgenstandardaufnahmen vor allem Schrägbilder. Die Operationen spielen sich in großer Tiefe ab und verlangen erheblichen Kraftaufwand. Die eigens entwickelte Beckenrepositionszange und das Repositorium nach ECKE erweisen sich dabei als außerordentlich nützlich und ermöglichen in manchen Fällen erst eine exakte Reposition. Wahl und Anordnung des Osteosynthesematerials werden ebenfalls bestimmt durch den Frakturtyp bzw. den Verlauf der Frakturlinien. Wesentlichen Anteil an Mißerfolgen trotz guter Rekonstruktionsergebnisse haben begleitende Femurkopfschäden mit nachfolgenden Nekrosen. Auch in solchen Fällen, in denen die Osteosynthese des Acetabulum nicht zum Dauererfolg führt, schafft die anatomische Rekonstruktion wesentlich günstigere Voraussetzungen für Arthrodesen oder endoprothetische Ersatzoperationen.
Summary
Compared with the unsatisfactory conservative therapy of fractures of the acetabulum, rather promising results are seen in such cases in which subluxations and incongruencies of the joints are the sole cause of functional and degenerative damages of the joints. Beyond that a series of local and general factors impair the results of therapy. The choice of access and operative tactics are decisive for the success of the difficult and extensive reconstruction. Knowledge of the fractureline and of the type of fracture have to be obtained not only by standard but also by oblique roentgenograms. The operations are performed in great depth and require considerable strength. The especially devised forceps for reposition of the pelvis and the instrument for reposition by ECKE have proved to be very usefull. In some cases an exact reposition is only possible by this means. Choise and order of the material for operative reposition and internal fixation are also determined by the type of fracture and the course of the fracture-line. Damages of the femoral head with consecutive necrosis plays an important role in those failures, in which good results of reposition have been obtained. Also in those cases in which operative reposition and internal fixation of the acetabulum does not lead to permanent success, the anatomical reconstruction of the joints gives a much better basis for arthrodesis and hip-replacement.
Résumé
A l’encontre de l’insatisfaisante thérapeutique conservative, l’ostéosynthèse des fractures de l’acétabule promet une nette amélioration des résultats obtenus pour les cas où la subluxation et les incongruités articulaires sont les seules origines des lésions articulaires. Toute une série de facteurs généraux et locaux entrave fatalement le résultat du traitement. Le choix des accès et de la tactique opératoire décident du succès des reconstructions complexes et étendues. Elles nécessitent la connaissance de l’évolution de la fracture et des différents types de fractures, et requièrent avant tout des radios en coupe plutôt que des radios-standard. Les opérations sont réalisées en grande profondeur et exigent une grande force. La pince de reposition du bassin et le repositorium créé par ECKE se révèlent la extrêmement efficaces: eux seuls permettent une reposition exacte dans certains cas. Le choix et l’agencement du matériel d’ostéosynthèse sont déterminés par le type de fracture, plus exactement par l’évolution des lignes de fracture. Les lisions concomitantes de la tête du fémur, avec les nécroses qui s’en suivent, sont les principales causes d’échec malgré de bons résultats de reconstruction. Même dans les cas où l’ostéosynthèse de l’acétabule ne mène pas à un résultat durable, cette reconstruction des articulations donne une base largement plus favorable pour l’arthrodèse et les replacements emprothétiques.
