Grundsätzliche Überlegungen zur Problematik der Abbrüche des Trochanter major bei und nach Winkelplattenosteosynthesen

Zusammenfassung

Intra- und postoperative Abbrüche des Trochanter major bei der operativen Versorgung von pertrochanteren Femurfrakturen sind keine Seltenheit. In der vorliegenden Arbeit werden die wichtigsten Ursachen dargelegt und Empfehlungen zur Vermeidung dieser Komplikationen gegeben. Ein wichtiger Kritikpunkt beim operativen Zugang ist die allgemein empfohlene, aber völlig überflüssige L-förmige Abtrennung des M.-vastus-lateralis-Ursprunges. Den entscheidenden Schwachpunkt stellt jedoch das für die Einbringung der Implantate notwendige große Knochenfenster in der lateralen Femurkortikalis dar.

Abstract

During the operative treatment of pertrochanteric fractures intra- and postoperative fractures of the trochanter major are wellknown. In this paper the most important reasons for this complication are explained and advices to avoid it are given. One main cause for this complication is the cutting off of the M. vastus lateralis during the operation as it is described in the most technical handbooks of surgery. The most important point of weakness is the big hole in the lateral femoral corticalis, where the blade of the angle-plate is inserted into the femoral neck and head.
