Phalloplastik mit dem osteokutanen Unterarmlappen

Zusammenfassung

Grundlagen: Das Fehlen des männlichen Gliedes ist auf einen Phallusverlust nach Trauma, Tumorgeschen, Infektion oder Selbstinjektion zurückzuführen. Auch Aplasie oder Hypolasie können eine Indikation für eine Phalloplastik sein.

Methodik: Anhand von 2 Fällen wird die Bildung des Phallus mittels osteokutanen Unterarmlappens, die Harnröhrenbildung mittels Harnblasenschleimhaut dargestellt.

Ergebnisse: Die Phalloplastik mittels osteokutanen Unterarmlappens führt zu einem sehr ansprechenden funktionellen und kosmetischen Ergebnis. Soweit derzeit beurteilbar, ist dieses Ergebnis anderen Techniken der Phalloplastik überlegen. Die Bildung der Neourethra mit Harnblasenschleimhaut stellt einen physiologischen Ersatz dar und ist nicht kompliziert durch Infektion oder Steinbildung, wie es bei Bildung der Neourethra durch Haut der Fall ist. Als Nachteil muß ein Höhleneingriff in Kauf genommen werden.

Schlußfolgerungen: Die Phalloplastik unter Verwendung des osteokutanen Unterarmlappens und Harnblasenschleimhaut führt zu dem derzeit besten funktionellen und kosmetischen Ergebnis.

Summary

Background: The missing of phallus in case of trauma, tumor, infection or selfinjection can indicate a phalloplasty. Aplasia or hypoplasia are further reasons where phalloplasty is necessary.

Methods: In 2 cases the phalloplasty with a free osteocutaneous forearm flap and the urethroplasty with bladder mucosa is showed.

Results: The phalloplasty by means of an osteocutaneous forearm flap leeds to a good functional and cosmetic result. The urethroplasty by means of bladder mucosa is a physiological replacement and is not complicated by infections and stoneformation because of hairs and scales in case of the use of skin graft. The disadvantage is the necessity of opening of the urine bladder.

Conclusions: The phalloplasty with the osteocutaneous forearmflap and with bladder mucosa results in the best functional and cosmetic outcome at time.
