Zusammenfassung
Grundlagen: Die Wiederherstellung durchgehender Bauchdeckendefekte kann schwierig sein. Bei sieben Patienten wurden große durch alle Wandschichten reichende Defekte mit myofaszialen Lappen wiederhergestellt.
Methodik: Vier Patienten hatten eine Bauchwandresektion wegen eines malignen Tumors, einmal trat eine nekrotisierende Bauchdeckenphlegmone nach Ovarialzystenentferung auf und einmal kam es zu einem Bauchdeckenverlust nach Schußverletzung und einmal nach Resektion einer Strahlennekrose. Sechsmal wurde als erste Schichte zur Rekonstruktion Corium oder Kunststoffnetz verwendet. Die Hautfaszien- und Muskelrekonstruktion konnte einmal mit einem kombinierten TFL und Rectus femoris-Lappen, einmal nur durch letzteren, viermal durch einen Rectus abdominis-Lappen und einmal durch einen LD-Lappen erreicht werden.
Ergebnisse: Die Verwendung von myofaszialen und myokutanen Lappen in Kombination mit Kunststoffnetzen oder Corium waren erfolgreich. Die Komplikationen waren minimal.
Schlußfolgerungen: Die Verwendung von gestielten oder mikrovaskulär transplantierten zusammengesetzten Lappen, speziell der M. rectus abdominis und M. rectus femoris-Lappen sollten bei der Rekonstruktion von ausgedehnten Bauchwanddefekten vermehrt eingesetzt werden. Die Vorteile sind die einfache Hebetechnik, die große Rotationsachse bei Insellappen und das große Areal, das man damit verschließen kann.
Summary
Background: Reconstructing full-thickness abdominal wall defects can be difficult. Assessment was made of the results of massive abdominal wall defect reconstruction using myofascial units in 7 patients.
Methods: Four patients had resection for malignancy, one patient had a necrosis after an operation, one after a gunshot injury and one after a resection of an irradiated area of the abdominal wall. The defect was closed with a mesh or dermis graft and myo fasciocutaneous units as extended rectus femoris TFL or rectus femoris flap only, extended rectus abdominis island flap or LD microvascular flap.
Results: Application of myofascial and myocutaneous flaps in combination with synthetic material or corium was successful. The complications encountered were minimal.
Conclusions: The use of myofasciocutaneous units should be considered the preferred method for repairing major abdominal wall defects. They are superior in their reliability, ease of use, arc of rotation, and potential area covered.
